of my knowledge the proposed design has been prepared in accordance with the applicable regulations, Gravity Sewer
Minimum Design Criteria for Gravity Sewers (latest version), and the Minimum Design Criteria for the Fast -Track Permitting
of Pump Stations and Force Mains (latest version). Although other professionals may have developed certain portions of this
submittal package, inclusion of these materials under my signature and seal signifies that I have reviewed this material and
have judged it to be consistent with the proposed design.
NOTE -- In accordance with General Statutes 143-215.6A and 143-215.613, any person who knowingly makes any false
statement, representation, or certification in any application package shall be guilty of a Class 2 misdemeanor, which may

The purpose of this Gravity and Sanitary Sewer Foccemain Relocation project (total of 6,078
LF) is due to the fact that NCDOT is improving US 70. The type of work included in this project
will be Grading, Drainage, Paving, Structures, Retaining Walls and Culverts. The existing
sanitary sewers will be in conflict with the new construction. The location of this work is along
US 70 from east of US 70 Business to west of Neuse River.
The improvements Include the construction of approximately 5,666 LF of 4", 6" and 10"
sanitary sewer forcemain and 412 LF of 12" gravity sanitary sewer. The forcemains will
be constructed with SDR 21 PVC and the gravity sewers will be constructed with
AWWA C-900 DR 18 Class 235 PVC and Class 50 RJ DIP (Protecto 401 Ceramic
Lined). This work will be performed in accordance with the NCDOT BMP for
Construction and Maintenance Activities dated August 2003.
The new relocated gravity sewers and forcemains will be owned, operated, and
maintained by Johnston County. All construction will be in accordance with the latest
standards and specifications of the Johnston County, NCDOT, North Carolina
Department of Transportation and the NCDEQ, Division of Water Resources, Water
Quality Permitting, PERCS Unit.

Dear Mr. Zaccardo:
This letter is to confirm that the engineering plans and technical specifications have been approved for the sewer
facilities to serve US 70 from east of US 70 Business to west of Neuse River (W-5600), Johnston County Serial No.
19-307-W.
Upon completion of the construction or modification, and prior to placing the new construction or modification
into service, the following information must be received and approved by the Johnston County Department of
Utilities:
1. NCDENR approval letter
2. Engineer's certification
3. As -Built drawings sealed by the Engineer
4. Lien waiver from the utility contractor
S. Recorded plat indicating any necessary utility easements
Please have the utility contractor schedule a pre -construction conference with our office prior to the installation
of the sewer distribution facilities. Please be advised that all permits must be received by this office prior to
