Normally we would have our VP of Manufacturing be the legally responsible person. However, he is leaving the
company within the next couple months and we do not know who his successor will be. Just let me know when you
need this complete. I can be if needed.
I'm surprised that Randy Winston was still a contact as I've renewed at least one permit and changed to a different
category permit since I've been here. Please remove and replace with my information. Address and office phone will be
the same.
I have updated the outfalls and printed the new permit.

Mr. Winston,
We were preparing to send renewal letters and noticed that we were missing information in our permitting database for
this facility in Statesville (covered under Stormwater NPDES permit no. NCG180256). We corrected the gap in
waterbody information and have generated the renewal Certificate of Coverage (COC) for you —please see attached.
Because waterbody info was missing, you cannot get a COC from the website as instructed in the renewal letter (also
attached). Please skip step #1 in the letter since you already have the COC. However, there are other items that need
attention.
• You will need to obtain a copy of the NCG180000 General Permit print package from our website. It includes a
copy of the new General Permit that became effective August 1, 2019.
• You will need to enter the stormwater outfall locations at this site with our website form (you will fill out
information about each one and locate it on a map).
• Finally, we do not have a legally responsible individual listed as the Owner Affiliation for this permit. We must
have a legally responsible person for all permit COCs. You can see Owner Affiliation in the attached report is
blank. Please let me know who the Owner Affiliation should be, along with email address and phone
number. Verify the address is correct.
If you have any questions, please let me know. A hard copy of the letter with the COC is being mailed to the PO Box
1842 address in Statesville. We have just mailed it to the attention of "Stormwater Permit Contact".

Dear Permittee:
Please see the attached new Certificate of Coverage No. NCG180256 for renewed coverage under Stormwater
General Permit NCG180000 for this facility. This permit is reissued pursuant to the requirements of North
Carolina General Statute 143-215.1 and the Memorandum of Agreement between the state of North Carolina
and the U.S. Environmental Protection Agency, dated October 15, 2007 (or as subsequently amended.)
You must print a copy of the new NCG180000 General Permit from our Stormwater General Permits web
page. In addition to the full permit, the most recent print package on the website includes revised Discharge
Monitoring Report (DMR) forms and Qualitative Monitoring Report Forms.
The General Permit authorizes discharges of stormwater, and it specifies your obligations with respect to
discharge controls, management, monitoring, and record keeping. Please review the new permit to familiarize
yourself with all changes in the reissued permit. Your facility has six (6) months from receipt of the permit to
update your Stormwater Pollution Prevention Plan (SPPP) to reflect any new permit requirements.
When does electronic DMR (eDMR) reporting start, and does it affect this permit?
If activities at this facility ever require analytical monitoring, eDMR will affect you. We are setting up our
database with the outfalls that permittees submitted on-line and permit parameters. Permittees will receive
notification when our eDMR system is ready for permit -holders to register and begin reporting monitoring data
electronically. If you have any questions about the status, please contact Bethany Georgoulias at (919) 707-
3641.
Your coverage under the General Permit is transferable only through the specific action of DEMLR. This
permit does not affect the legal requirements to obtain other permits which may be required by the N.C.
Department of Environmental Quality (DEQ), nor does it relieve the permittee from responsibility for
compliance with any other applicable federal, state, or local law, rule, standard, ordinance, order, judgment, or
decree.
