Kaymoore Dam is a small, earthen dam located in a residential area behind 160 Lochmoore Court in Winston Salem, NC. The dam impounds a small pond on the HOA owned property
that includes a playground and short walking path. The dam owner has been issued a Notice of Deficiency from the NC Dam Safety Office citing deficiencies in the structure that
jeopardize the safety of the dam. The dam and area downstream of the dam are grass covered, maintained and mowed by the HOA and homeowners, so there are no wetland areas
within this project.

4h. Explain the purpose of the proposed project:*
The purpose of this project is to repair the deficiencies listed in the state's Notice of Deficiency to the dam owner. This project will provide erosion protection on the upstream slope of
the dam and at the outlet discharge location. A toe drain will be added to the downstream slope to protect against damage from seepage. Because this dam is classified as "High
Hazard", the repair project will have to also include additional outlet capacity to safely pass the 1/3 PMP design storm. Additional capacity will be added by installing a larger riser/barrel
system.
4i. Describe the overall project in detail, including indirect impacts and the type of equipment to be used:*
This project will replace a small brick riser and 4" outlet with a 6'x 8' concrete riser and 48" barrel to provide outlet capacity to pass the 1/3 PMP design storm for a high hazard dam. The
downstream slope will be extended to provide a 3 to 1 slope and a toe drain will be installed along the toe of the slope. An appropriately sized energy dissipator will be added at the
outlet location, where erosion is occurring. Construction drawings are being prepared to provide these dam repairs, and the 90 % complete plans are included in this submittal for your
review.
Construction practices will follow guidelines from the NC Erosion and Sediment Control Planning and Design Manual, and there is an Erosion and Sediment Control sheet included in the
plans. The energy dissipator will result in a permanent impact to the stream, but all other impacts of construction will be temporary. The area will be stabilized with permanent seeding
appropriate for the area.

KAYMOORE DAM REPAIRS
Introduction
Kaymoore Dam is a small, earthen, high hazard dam located in a residential area behind 160
Lochmoore Court in Winston-Salem. The dam owners have been issued multiple inspection
reports and a Notice of Deficiency (NOD) from the NC Dam Safety office citing deficiencies in
the structure that jeopardize the safety of the dam.
Kaymoore Dam (the Project Site) is located in the Kaymoore Subdivision, in the southwest
quadrant of Winston-Salem, near Old Salisbury Road and Darwick Road. Flow enters the stream
from overland flow from an 18 acre watershed, as shown on Figure 1. Flow discharges from the
pond outlet to an unnamed tributary that flows to South Fork Muddy Creek. This project is in
Forsyth County and Figures 2 and 3 show the project area on a county soils map and a USGS
topographical map.
Because this dam is classified as high hazard, this project needs to add outlet capacity and
provide seepage control in order to bring the dam up to current state regulations.
Improvements and Repairs
This project will replace a small brick riser and 4" outlet with a 6'x 8' concrete riser and 48"
barrel to provide outlet capacity to pass the 1/3 PMP design storm for a high hazard dam. The
downstream slope will be extended to provide a 3 to 1 slope and a toe drain will be installed
along the toe of the slope. An appropriately sized energy dissipator will be added at the outlet
location, where erosion is occurring. Construction drawings have been prepared to provide these
dam repairs.
This project constitutes a positive impact by increasing outlet capacity to reduce the risk of
overtopping and providing energy dissipation at the outlet to protect against erosion during
significant storm events. Construction practices will follow guidelines from the NC Erosion and
Sediment Control Planning and Design Manual.
Erosion and Sediment Control Measures
Erosion control methods to be used in the project will include construction entrances/exits,
draining the lake, pump -around operation, temporary sediment trap, silt fences, mulch, temporary
seeding, and permanent seeding.
When working in the stream, the Contractor will be permitted to disturb only as much streambank
as can be stabilized with seeding, mulch and erosion control matting by the end of each work
week or prior to expected substantial rainfall.
Excavation on the dam will be accomplished from the downstream side of the embankment. Soil
will be stockpiled in locations shown on the plans and be surrounded on three sides with silt fence
to keep the soil from migrating to the stream. The downstream slope of the dam will be expanded
to provide a 3 to 1 slope, so excavated soils are expected to be used on site. Unusable material
and debris will be hauled off -site. Weekly and post -rainfall inspection and maintenance of silt
fences will be required.
All construction will take place in the dry. A temporary pump around operation will be used to
divert flow during construction activities.

Subject: Consistency letter for the 'Kaymoore Dam Repair' project indicating that any take of
the northern long-eared bat that may occur as a result of the Action is not prohibited
under the ESA Section 4(d) rule adopted for this species at 50 CFR § 17.40(o).
Dear Amy Black:
The U.S. Fish and Wildlife Service (Service) received on September 06, 2019 your effects
determination for the 'Kaymoore Dam Repair' (the Action) using the northern long-eared bat
(Myotis septentrionalis) key within the Information for Planning and Consultation (IPaC)
system. You indicated that no Federal agencies are involved in funding or authorizing this
Action. This IPaC key assists users in determining whether a non -Federal action may cause
"take"L11 of the northern long-eared bat that is prohibited under the Endangered Species Act of
1973 (ESA) (87 Stat.884, as amended; 16 U.S.C. 1531 et seq.).
Based upon your IPaC submission, any take of the northern long-eared bat that may occur as a
result of the Action is not prohibited under the ESA Section 4(d) rule adopted for this species at
50 CFR § 17.40(o). Unless the Service advises you within 30 days of the date of this letter that
your IPaC-assisted determination was incorrect, this letter verifies that the Action is not likely to
result in unauthorized take of the northern long-eared bat.
Please report to our office any changes to the information about the Action that you entered into
INC, the results of any bat surveys conducted in the Action area, and any dead, injured, or sick
northern long-eared bats that are found during Action implementation.
If your Action proceeds as described and no additional information about the Action's effects on
