Please note that I am a Register Professional Engineer and that I have been involved with plan reviews (in some cases the design) and field inspections during the period covered by the seven permits. Also, the City employs a City Inspector and during the same period, and his responsibilities are to provide field inspection of the construction and to witness testing including air testing, mandrel testing and TV inspection. At this time, the City has not experienced any problems regarding the permitted sewers operation and we have accepted all maintenance responsibilities for the extension systems. WQ0021419 — Stowe Manor Subdivision On July 15, 2011, I sent correspondence to Dee Browder at the Morresville Regional Office regarding the completion of the sewer extension project. In addition to my letter to Ms. Browder, I have included my inspection reports for the manholes and limited TV inspection results. The City Inspector witnessed the construction and testing of the system. Attached are the following: • Signed Sewer Extension Permit Status Update Form • Signed Fast Track Sewer Engineering Certification Form • Copy of letter to Ms. Browder with inspection reports WQ0022831 —West Outfall Sewer Replacement The permit for this City project was issued as a separate project before the West Outfall work was added to a much larger sewer construction project. I was the engineer for Christyn L. Fertenbaugh October 29, 2019 both projects, and witnessed the construction. I apparently overlooked that both projects had separate permits and I only certified the larger project permit. As with all sewer construction in the city, our City Inspector provided periodic inspection during construction and witness all testing. Attached are the following: • Signed Sewer Extension Permit Status Update Form • Signed Fast Track Sewer Engineering Certification Form • As-Built Plans WQ0023254 — Gaston County Visitor's Center This project has been completed, but we do not have as-built plans for it. Our City Inspector provided periodic inspection during the construction and witnessed all testing. The sewer system has been in operation for several years without any problems. Attached are the following: • Signed Sewer Extension Permit Status Update Form • Signed Fast Track Sewer Engineering Certification Form WQ0023379 — South Point Ridge Subdivision This project has been completed, and we do have as-built plans for it certified by the design engineer. Our City Inspector provided periodic inspection during the construction and witnessed all testing. Our City Inspector provided periodic inspection during the construction and witnessed all testing. I was the design engineer and it went to construction. As with all of the other projects, our City Inspector provided periodic inspection during construction and witness all testing.
