Accordingly we are forwarding the attached, corrected permit page. Please remove the existing "Effluent Limitations and Monitoring Requirements" page and replace it with the one attached to this letter. The correction affects effluent monitoring for chloroform, changing the sample type from composite to grab. This modification is issued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the U.S. Environmental Protection Agency dated October 15, 2007 (or as subsequently amended). Please note that this permit is not transferable except after notice to the Division. The Division may require modification or revocation and reissuance of the permit. This permit does not affect the Iegal requirements to obtain other permits which may be required by the Division of Water Resources or any other Federal, State, or Local governmental permits that may be required. No later than December 1, 2014 (270 days from the effective date of this permit), begin submitting discharge monitoring reports electronically using NC DWR's eDMR application system. See Condition A. (3.). 2. Sample locations: E-Effluent; I -Influent; U-Upstream 50 feet above the outfall; D-2.8 miles below the outfall at the mouth of Little Mountain Creek. Instream monitoring is provisionally waived in light of the permittee's participation in the Yadkin Pee Dee River Basin Association 3. The facility shall report on a monthly basis flow entering the collection system from the Alcoa / Badin Landfill near Wood Street. This flow measurement shall be submitted with facility December DMR as a separate inflow to the treatment plant in million gallons per month. 4. The monthly average effluent BODS and Total Suspended Solids concentrations shall not exceed 15% of the respective influent value (85% removal). 5. The Division shall consider all effluent TRC values reported below 50 ug/1 to be in compliance with the permit. However, the Permittee shall continue to record and submit all values reported by a North Carolina certified laboratory (including field certified), even if these values fall below 50 ug/l. 6. Monitoring should be performed in conjunction with toxicity testing. 7. Chronic toxicity (Ceriodaphnia dubia) 7 day pass/fail test at 77%: January, April, July, and October. See Condition A. (2.). 8. See Condition A. (4.) for monitored parameters and monitoring frequency. There shall be no discharge of floating solids or visible foam in other than trace amounts. Page 3 of 9
