Division of Water Resources' Central and Regional staff has reviewed the application package
received August 13, 2019. However, additional information is required before the review may be
completed. Please address the items on the attached pages no later than the close of business on December
2, 2019.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. hi addition, any omissions made in responding to the outstanding items,
or failure to provide the additional information on or before the above requested date may result in your

A. Application (15A NCAC 02T .0105):
1. Provide an agreement between Carolina Water Service and Belvedere Utility Company (owner of the
property on which the treatment facility is located) allowing for the treatment of wastewater on land not
owned by the Permittee.
2. Provide a map showing the 57.53 acres of reclaimed water irrigation. The map should include property
lines, surface waters, surface water drainage controls, groundwater wells, and the wetted perimeter of the
irrigation area.
3. Provide a property line setback waiver for the treatment unit. The treatment unit is less than 50 feet away
from the property owned by DMB Belvedere LLC.
4. The Agreement for Disposal of Treated Effluent does not meet all the easement requirements per 15A
NCAC 02L .0107(f):
a. Prohibits the construction and operation of water supply wells within the compliance boundary.
b. Reserves the right to the permittee and the State to enter on such property within the compliance
boundary for groundwater monitoring and remediation purposes.
