Subject: Notice of Violation: NOV-2019-LV-0759
Fairfield Harbour W WTP
NPDES Permit NC0033111
Dear Mr. Tankard:
In reference to the Notice of Violation dated 16 October 2019, we offer the following response:
The ultraviolet (UV) disinfection system bulbs were replaced the week prior to the Enterococci
exceedances as a part of routine maintenance. Upon notification of the high results, an investigation
revealed that the new bulbs had been stored in our warehouse when the building was flooded by
Hurricane Florence. Though there was no evidence of damage to them, the working intensity
measured at only 51 %. Operators immediately converted to the back-up chlorine disinfection system
and ordered new bulbs. All subsequent samples were well within permit compliance. The UV
system was brought back online on 25 June 2019.
Respectfully,
Dana Hill
