W DESCRIPTION OF WORK Project purpose: Replacement of structurally deficient bridge with a new bridge. Existing structure: Bridge 143 consists of 1@ 34.9', 1@ 45', 1@ 35' and 1@ 44.9' timber floor on I -Beams with timber/concrete caps and piles. Proposed structure: 2@ 45', 2@ 55' 21" cored slab with 4' caps. IMPACT SUMMARY Permanent: 0.10 permanent surface water impacts Temporary: 0.41 temporary surface water impacts Is mitigation proposed for project?: No, mitigation is not proposed for impacts on this project. PROTECTED SPECIES INFORMATION Northern long eared bat: BC of May Affect, Likely to Adversely Affect based on programmatic biological opinion with the FHWA, NCDOT and USACE that allows for incidental take for NLEB for projects with a federal nexus in Division 5. Dwarf wedgemussel: Use of the Programmatic Biological Opinion for Bridge and Culvert Replacements/Repairs/Rehabilitations in Eastern North Carolina, NCDOT Divisions 1-8 (June 13, 2018) and a Protected Aquatic Species Assessment (June 5, 2019) indicates a BC of No Effect for this species due to lack of suitable habitat. Atlantic pigtoe: BC of No Effect following information from Protected Aquatic Species Assessment (June 5, 2019) due to lack of suitable habitat. Yellow lance: Use of the Programmatic Biological Opinion for Bridge and Culvert Replacements/Repairs/Rehabilitations in Eastern North Carolina, NCDOT Divisions 1-8 (June 13, 2018) and a Protected Aquatic Species Assessment (June 5, 2019) indicates a BC of No Effect for this species due to lack of habitat. Smooth coneflower: Suitable habitat was observed during a site visits on May 18, 2017 and May 31, 2019. No individuals of this plant species were observed. BC of No Effect is provided for this species. Bald eagle: BC of Unlikely to Adversely Affect based on presence of feeding, roosting and nesting habitat at site but no observations of nests during the site visit on May 18, 2017 and May 31, 2019. Carolina madtom: BC of No Effect following information from Protected Aquatic Species Assessment (June 5, 2019) due to lack of suitable habitat. Neuse River waterdog: BC of No Effect following information from Protected Aquatic Species Assessment (June 5, 2019) due to lack of suitable habitat. Harperella: BC of No Effect based on lack of riverine habitat at site CULTURAL RESOURCES INFORMATION Historic Architecture: Historic Architecture and Landscapes No Survey Required Form dated March 27, 2017. PERMIT REQUESTED: Nationwide Permit 3 KERR LAKE WATERSHED The replacement of the bridge results in a net gain of watershed volume below the Kerr Lake Full Pool Land Acquisition elevation of 321 feet mean see level. A non -recreation out grant application for temporary and permanent easements was provided to Shannon Crews (USACE) via email on March 12, 2019. The NCDOT provided the Mitigation Proposal to Shannon Crews (USACE) via email on March 15, 2019. The Department has been advised that the USACE has approved the final easement and the only remaining action is to record the information.
