Dear Mr. Clark,
On October 16, 2019, Kelli Park and Mike Turner, of this office, met with Eric Bolick, Operator in
Responsible Charge (ORC), to perform a Compliance Evaluation Inspection at the Bethel
Elementary School wastewater treatment plant. This type of inspection consists of two basic
parts: an in -office file review and an on -site inspection of the treatment facility. The attached EPA
inspection form details the areas that were evaluated during this inspection.
