Information Processing Unit:
The Town of Newport received a Notice of Deficiency on April 20, 2015 regarding the failure to send and
2014 Annual Report for Permit WQ0035628. No distribution of Class A material was performed under
this permit and The Town of Newport believed that with the April,25, 2014 letter to rescind this permit,
the permit had if fact been rescinded and no annual report was due. All waste from the Town of
Newport's WWTP were applied under the conditions of permit WQ0008349 which has had an annual
report filed in January 2015. All monitoring data for this facility was submitted with the annual report for
WQ0008349. Attached are both the April 25, 2014 rescission and a new April 24, 2015 rescission letter.
Please forward this information to the appropriate regional office. Please respond back that the letter to
rescind permit WQ0035628 has been processed.
Attached is the Distribution Certification Form indicated no distribution under this permit.
