vegetation clearing, buildings, retaining walls, docks, impervious surfaces, rip rap,
excavation or dredging below Full Pond/ Normal Water Level elevations, and
construction access corridors. You may .use the diagram provided at the end of this
application form.

Starting on.Main. Street heading South In the town of Catawba:
Get, into the left lane where it intersects with 2nd Street at the: stop sign. Turn Left following
.2nd Street.for .7 mile until it dead.ends at HudsonChapel Road. Turn Left onto Hudson
Chapel Road and continue 2.2 miles. Shortly after passing Hudson Chapel Church on your
left ,you will turn Left onto Glovier Road. Follow Glovier for one mile. Glovier starts out as
a narrow asphalt road which changes into a gravel road Take the left fork when the rg avel
road splits At the bottom of the hill Glover Road. intersects with Zelda Drive. Look -on your_
left for 6774 Zelda Dr.... its the third house on your left.

It is a residential neighborhood.
6. Describe proposed work (include discussion as to.how hardening of shoreline has been avoided, or
why it is necessary):
The plan is to install rip rap along the 100 feet of river embankment behind our home. Neighbors, on
both sides -of the property haverip rap along the. riverbank but unfortunately our lot does not.
Increased boating activity (waves) over recent years have caused our embankment to erode. Recent
major storm damage (June 2019) from water flow/water pressure has resulted in tree roots being
undercut leaving them exposed and the tree trunks severely leaning. We would like to remove the
damaged trees -and install enough riprap to stabilize the bank. Our back yard extends from our
house to the river so there would be no impact on or removal of established wild vegetation.
Hardening of the shore line would result from added weight of rip rap and use of equipment to
install the rip rap Above full pond hardening would occur from use of mobile equipment.

7. Will work be conducted from land or from water?
Our intent is to do the mja pg Yof work from land. But the.base stones will be too hgM to
manually lift thusrequiring a mechanical_ lift to be used either from a boat or from. land.
8. Total amount of impacts below the normal pool lake level/ Normal Water Level
Permanent impacts (including all excavation, backfill, rip rap, retaining walls, etc.) mi square feet:
Three'hundred square feet of rip rap will be installed below the normal water level "to prevent
erosion and maintain bank integrity. Heavy g eotextile fabric will be installed under the rip rap to
aide in erosion control.
Temporary impacts in square feet:
Installation of rip rap should be completewithin a week of starting this phase of the project.. There
should be no tempgM impacts below the normal pool level. .
9. Total amount of impacts, above the normal. pool lake level/:Normal• Water Level and 50 feet
land -ward:
Permanent impacts (including: back fill, excavation, rip rap; retaining walls,. etc.) in square feet:.
installed under the rip rap to minimize soil erosion. and/or compaction. Movement of soil will be
held to a minimum as the rip rap and fabric are installed. The fabric will be placed over oy grass or
vegetation currently growing on the embankment and.rip rap will be placed -over the fabric to ensure
soil retention and prevent stone movement. 2) Any soil loss or hole created due to approved tree
removal will be replaced/repaired with new soil and manually_ compacted to even out the effected
area. We request to remove one dead tree and any undercut tree that can not be properly/safely
re-established.
Temporary impacts (temporary clearing, construction corridor) in square feet:
Material will be delivered in large dump trucks via the driveway on the West (left) side of the lot
and travel 300 feet to the riverbank. Entry from the East side would be difficult because the septic
system is located just behind the East side of the house. Tire ruts and paths will have to be repaired
as part of the job completion. Total area which may be affected is: estimated to be 6,750 square feet..
This results _from trucks/machinery making multiple trips over the, left side of the property
(35' -x 501 =-1,750sf) and trucks moving: and,turning along the 100 ft river frontage (100' x 50' _
5000sf). All of this travel will be across an established grass/yard.

10. Please describe the vegetation above the normal pool lake level/ Normal Water Level and 50 feet
landward to be impacted (number of trees, for instance):
The yard (pass) and trees cover from the house to the normal lake level. On the right (East) side
we have two large trees. One is a "boundary tree" that serves to delineate our property from our
neighbors Also on the right side we have a large dead tree that we are requesting for permission to
remove. On the left side we have trees that have been undercut by boat waves and the ravages of the
June 2019 storm It should be noted that there are stubs of several bushes shrubs or small trees that
the previous owner attempted to cut down over three years ago.
