On September 17, 2019, Shelton Sullivan of DWR, conducted an on - site determination to review the feature labeled as "Feature A", an unnamed tributary to Crooked Creek, for applicability to the Neuse River Riparian Area Protection Rules 15A NCAC 02B .0233. During the review, Mr. Sullivan evaluated "Feature A" using the DWR Stream Classification Form and the results are below and photographs of these locations are attached. This determination shall expire five years from the date of this letter. This determination can be contested as provided in General Statute 150B by filing a written petition for an administrative hearing to the Office of Administrative Hearings (hereby known as OAH) within sixty (60) calendar days. A petition form may be obtained from the OAH at http://www.ncoah.com/ or by calling the OAH Clerk's Office at (919) 431-3000 for information.
