Dear Permittee:

I conducted an inspection of the Brookside Village Condos WWTP on 10/10/2019. I was surprised to learn
from Mark Jones (ORC) that the tertiary filters, which are essential treatment units at the WWTP, are
completely non-functional. I will schedule a follow up inspection of this facility when the pumps have been
repaired/replaced and the tertiary filters are in operation again. Until the repairs are complete,
NOD-2019-PC-0189 will remain in effect, which was issued for the previous inspection of the facility. For
details of this inspection, please see the enclosed inspection report.
