Authorization is hereby granted for the construction of improvements to upgrade the existing Town of Spindale 6.0 MGD Wastewater Treatment Plant with no increase in flow capacity to consist of a new waste sludge pump, removal of old clarifier weirs, installation of new clarifier weirs, adjustment of the existing 110' clarifier weir to level, removal of the existing grit pump and grit separator, installation of a new grit screw to operate with the existing grit chamber, replacement of sluice gates with slide gates in the existing diversion box, replacement of existing post aeration air piping, and all related appurtenances. In addition, a new 36-inch diameter, 6,600 linear foot effluent outfall line from the Spindale Wastewater Treatment Plant to Catheys Creek will be constructed. Do not proceed with construction until the Authorization -to -Award package and EEO and MBE documentation/certification have been reviewed, and you are in receipt of our approval, if funding is desired for project construction. The Permittee shall employ an actively State certified wastewater treatment plant operator to be in responsible charge of the wastewater treatment facilities. Such operator must hold a valid State Certificate of the grade at least equivalent to the classification assigned to the wastewater treatment facilities by the Certification Commission. When a new chief operator must be obtained to be in responsible charge of the new facilities, it is required that this be done by the time the construction of the facilities is 50% complete. The Permittee shall have an acceptable sludge analysis and a sludge toxicity analysis which have been approved by the Division of Water Quality prior to the disposal of any sludge from the new wastewater treatment plant. Copies of the analyses, supporting documents, and approvals shall be submitted to the Division of Water Quality's Asheville Regional Office. The disposing, selling or free distribution of wet, dried, composted sludge or sludge -like materials from wastewater facilities without prior review and approval by this Division is illegal. During the construction of the proposed additions/modifications, the permittee shall continue to properly maintain and operate the existing wastewater treatment facilities at all times, and in such a manner, as necessary to comply with the effluent limits specified in the NPDES Permit. You are reminded that it is mandatory for the project to be constructed in accordance with the North Carolina Sedimentation Pollution Control Act, and, when applicable, the North Carolina Dam Safety Act. In addition, the specifications must clearly state what the contractor's responsibilities shall be in complying with these Acts. One (1) set of final approved plans and specification is to be forwarded to you for your files. The copy of the approved plans and specifications shall be maintained on file by the Owner for the life of the project. Upon completion of construction and prior to operation of this permitted facility, a certification must be received from a Professional Engineer certifying that the permitted facility has been installed in accordance with the approved plans and specifications. Authorization is hereby granted for the construction of a new effluent outfall line for the Spindale WWTP with final discharge of treated wastewater into Catheys Creek which is classified C waters in the Broad River Basin. This Authorization to Construct is issued in accordance with Part III, Paragraph A of NPDES Permit No. NCO020664 issued March 13, 1995, and shall be subject to revocation unless the wastewater treatment facilities are constructed in accordance with the conditions and limitations specified in Permit No. NC0020664. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the Permittee shall take immediate corrective action, including those as may be required by this Division, such as the construction of additional or replacement wastewater treatment or disposal facilities. The Asheville Regional Office, telephone number (704) 251-6208, shall be notified at least forty- eight (48) hours in advance of operation of the installed facilities so that an in -place inspection can be made. Such notification to the regional supervisor shall be made during the normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State Holidays. Upon completion of construction and prior to operation of this permitted facility, a certification must be received from a professional engineer certifying that the permitted facility has been installed in accordance with the NPDES Permit, this Authorization to Construct and the approved plans and specifications. Mail the Certification to the NPDES Unit, P.O. Box 29535, Raleigh, NC 27626-0535. A copy of the approved plans and specifications shall be maintained on file by the Permittee for the life of the facility. S. 143- 215.6 for violation of or failure to act in accordance with the terms and conditions of this Permit. Failure to abide by the requirements contained in this Authorization to Construct may subject the Permittee to an enforcement action by the Division of Water Quality in accordance with North Carolina General Statute 143-215.6A to 143-215.6C. The issuance of this Authorization to Construct does not preclude the Permittee from complying with any and all statutes, rules, regulations, or ordinances which may be imposed by other government agencies (local, state, and federal) which have jurisdiction. Authorization is hereby granted for the construction of dechlorination facilities, using sulfur dioxide gas, consisting of a combined bulk storage and chemical feed facility for two sulfur dioxide tank cylinders and three chlorine tank cylinders, gas metering and injection system separate from the chlorination gas metering and injection system, a 40 GPM feedwater pump, the existing chlorine contact effluent chamber with new over/ under baffles, a sulfur dioxide manifold, and associated piping, valves, controls, and appurtenances with discharge of treated wastewater into Holland Creek classified Class "C waters. This Authorization to Construct is issued in accordance with Part III paragraph C of NPDES Permit No. NCO020664 issued October 1, 1988, and shall be subject to revocation unless the wastewater treatment facilities are constructed in accordance with the conditions and limitations specified in Permit No. NC0020664. The sludge generated from these treatment facilities must be disposed of in accordance with G.S. 143-215.1 and in a manner approved by the North Carolina Division of Environmental Management. Authorization is hereby granted for the construction of a 750,000 gallon sludge holding tank, tanker loading area, and related appurtences. These modifications will replace the belt press dewatering system (equipment, structures, diffused air system at the sludge holding tank and appurtences) previously authorized for construction on March 2, 1988. This Authorization to Construct is issued in accordance with Part III, paragraph B of NPDES Permit No. NCO020664 issued September 29, 1.988, and shall be subject to revocation unless the wastewater treatment facilities are constructed in accordance with the conditions and limitations specified in Permit No. NC0020664. The Permittee must employ a certified wastewater operator. in accordance with Part III paragraph C of the referenced permit. The sludge generated from these treatment facilities must be disposed of in accordance with General Statute 143-215.1 and in a manner approvable by the North Carolina Division of Environmental Management. Permits must be obtained before the sludge is land applied." In the event the facilities fail to perform satisfactorily in meeting its NPDES permit effluent limits, the Permittee shall take such immediate corrective action as may he required by this Division, including the construction of additional wastewater treatment and disposal facilities. Authorization is hereby .granted for the construction of improvements to an existing 4.0 MGD. wastewater treatment facil.iLy consisting of two (2) new bar screens, aeration basin mixers and weirs, a 110-toot diameter clarifier, replacement of recirculation pumps, sludge digester diffusers, and sludge filter press system, replacement chlorinators, and a 92,300 gallon chlorine contact, post aeration and dechlorination chamber. This Authorization to Construct is issued in accordance with Part III paragraph B of the NPDE.S Permit No. NC0020664 issued July 1, 1987, and shall be subject to revocation unless the wastewater treatment facilities are constructed in accordance with the conditions and limitations specified in Permit No. NC0020664.
