Dear Mr. Judd:
A letter of request for Authorization to Construct was received October 11,
1985, by the Division and final plans and specifications for the subject project
have been reviewed and found to be satisfactory. Authorization is hereby
granted for the construction of stormwater collection facilities with capacity
of handle woodyard runoff from a 10 year-24 hour storm without overflow, said
facilities consisting of a 116,000 cu.ft. capacity stormwater holding basin
with ccn-patted clay liner and dual 500-GPM effluent pumps discharging through
a 6-inch force main into an existing emergency wastewater holding pond at
the Champion International Corporation's Roanoke Rapids Mill Wastewater Treat-
ment Plant.
This Authorization to Construct is issued in accordance with Part III Para-
graph C of NPDES Permit No. NC0000752 issued December 5, 1983, and shall be
subject to revocation unless the wastewater treatment facilities are constructed
in accordance with the conditions and limitations specified in Permit No. NC0000752.
The Permittee must employ a certified wastewater operator in accordance with
Part III paragraph D of the referenced permit.
The sludge generated from these treatment facilities must be disposed of in
accordance with G.S. 143.215.1 and in a manner approvable by the North Carolina
Division of Environmental Management.

The Raleigh Regional Office, telephone number 919/733-2314 shall be notified
at least twenty-four (24) hours in advance of start-up of the installed system
so that an in -place inspection can be made of said system prior to operation.
Such notification to the Regional Supervisor shall be made during the normal
office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding
State Holidays.
In event the facilities fail to perform satisfactorily in meeting its NPDES
permit effluent limits, the permittee shall take such immediate corrective
action as may be required by this Division, including the construction of ad-
ditional wastewater treatment and disposal facilities.
Freeboard in the stow water holding basin shall not be less than two (2)
feet at any time.
The clay lining for the stormwater holding basin shall be 12-inches minimum
thickness and shall have a permeability no greater than 1 x 10-6 cm/sec after
ccmpaction.
Samples of stormwater shall be collected from the basin three times,_a year, with
no two consecutive samples being taken at intervals of less than two (2) months.
The samples shall be analyzed for NO3, Fecal Coliform, Total Coliform, Fe, Total
Organic Carbon, pH, Cl and Phenol.
In the event that the concentration of any contaminants) in the stormwater
reach levels which may be considered to pose a potential danger to classified
groundwaters, the applicant may be required to install two or more groundwater
monitoring wells, with well location and construction, sampling schedules, and
sampling parameters determined by the Division of Enviromental Management.
