I Shuyan Tian certify to the best of my knowledge that the addition of
the volume of wastewater to be permitted in this project has been evaluated along the route to the receiving
wastewater treatment facility and that the flow from this project is not anticipated to cause any capacity
related sanitary sewer overflows or overburden any downstream pump station en route to the receiving
treatment plant under normal circumstances, given the implementation of the planned improvements
identified in the planning assessment where applicable. This analysis has been performed in accordance
with local established policies and procedures using the best available data. This certification applies to
those items listed above in Sections I and II plus all attached planning assessment addendums for which I
am the responsible party. Signature of this form certifies that the receiving collection system or treatment

We are requesting review and approval of a new on -site collection and pump system to service three
industrial buildings, housing forty-eight (48) employees. Each of the buildings will separately gravity
feed to a new collection manhole with duplex grinder Cum s outleting to 2" PVC forcemain, ultimately
discharging into Town of Cary pub 1c sewer main. All uildings being serviced are under same
ownership and are on a single parcel of land.
No Flow Tracking or Acceptance letter is required from the Town of Cary and no Certificate of Public
Convenience and Necessity is required, as this system will service a single user.
System will service a single user on a single lot; therefore, no Owners Association or Operational
Agreement is required.

Subject: Waste Management fast track sewer extension application
Blair and John,
We received the subject application 8-30-19 and have a couple of questions.
Please confirm the correct legal name of the applicant. The application noted 'Waste Management' as the
applicant. The NC Secretary of State's website has a listing of `Waste Management of the Carolinas, Inc'. Please provide
the correct legal name.
Also, John Workman, Senior Engineering Manager, signed for the applicant. Mr. Workman is not listed as an officer of
the company on the NC Sec. of State website. Has he been delegated to sign for the applicant?

15A NCAC 02T .0106 SUBMISSION OF PERMIT APPLICATIONS
(b) Permit applications shall be signed as follows:
(1) in the case of corporations, by a principal executive officer of at least the level of vice-president or his
authorized representative:
(c) Delegation of authority to sign permit applications to other authorized employees or any employee in a specific
position shall be provided in writing to the Division and signed by an authorized person pursuant to Paragraph (b) of this
Rule. The delegation may be for a specific permit application or for certain or all types of water quality permits. The
letter shall identify the extent of delegation.
