Enclosed is a copy of the Compliance Evaluation Inspection Report for the inspection conducted at the subject facility on
September 25, 2019, by Mr. Wes Bell of this Office. Please inform the facility's Operator -in -Responsible Charge (ORC) of
our findings by forwarding a copy of the enclosed report.
It is requested that a written response be submitted to this Office by November 6, 2019, addressing the concerns regarding
the old bar screen and V-notch weir angle, as discussed further in the Bar Screen and Flow Measurement — Effluent
Sections in the attached report. The response must include a copy of the third parry evaluation of the V-notch weir and any
corrective actions. Please address your response to the attention of Mr. Bell.
The report should be self-explanatory; however, should you have any questions concerning this report, please do

FLOW MEASUREMENT — EFFLUENT SECTION cont'd:
The V-notch weir angle was measured at approximately 83 degrees during the inspection; however,
the flow meter was programmed to measure flows for a 90 degree V-notch weir. The Permittee must
contract a third party experienced in flow meter calibrations to verify the V-notch weir angle and the
calibration procedures. Please be advised that the NPDES Permit requires the flow measurement
device be installed, calibrated, and maintained to ensure that the accuracy of the measurements is
consistent with the accepted capability of the type of device and shall be capable of measuring flows
with a maximum deviation of less than 10% from the true discharge rates throughout the range of
expected discharge volumes (Permit Condition Reference: Part II, Section D.3 Flow Measurements).

Monitoring Reports (eDMRs) were reviewed for the period August 2018 through July 2019.
No effluent limit violations were reported and all monitoring frequencies were correct.
The ORC and staff must ensure not to report (on edmrs) effluent total residual chlorine
(TRC) values below the lowest standard verified on the TRC meter (annual curve
verification).

Comment: The old coarse bar screen had oartiallv separated from the WWTP structure and was bein
held in place with a rope (in aeration basin). The old bar screen should be removed as soon
as possible to prevent this structure from breaking off into the aeration basin and potentially
damaging the aeration system (diffusers, air headers, etc.).
