MEASURES TO THIS APPLICATION,
AGREEMENT
1, the undersigned applicant, request permission to construct a driveway access on a City of Conover public street right-
of-way, and/or will be conducting land disturbing activities at the above location. I agree to the following:
® Construct and maintain the driveway in absolute conformance with the City Ordinance and conduct land -
disturbing activities in accordance with the North Carolina Sedimentation Pollution Control Act of 1973.
® Install drainage pipe of no Iess than 15" diameter 16 gauge corrugated metal or 15" reinforced concrete. In some
cases, a larger size pipe may be required due to drainage conditions as determined by the City.
L Include any approach tapers, storage lanes or speed change lanes that are deemed necessary.
® Locate the driveway access a minimum, of 50' firom any street intersection (locating a driveway access < 50' from
a street intersection shall only be approved at the discretion of the City).
Ll If any future improvements to the roadway become necessary, the portion of the driveway located in the public
right-of-way will be considered the property of the City of Conover and my property will not be entitled to
reimbursment or have any claim for present expenditures for driveway construction.
Ll Properly install and maintain sediment and erosion control best management practice(s) in accordance with "The
North Carolina Sediment and Erosion Control Design Manual' and in the location as shown on the sketch
attached hereto. I have received the City of Conover informational erosion and sediment control packet that
illustrates effective erosion and sedimentation control measures commonly used.
® Properly install and maintain a temporary construction drive (consisting of 2" diameter or greater stone) for this
location. This drive will be the sole point of access and no sediment will be deposited on the public street.
® Should sediment loss occur from this site, (such as vehicles depositing sediment in street) a written notice of
warning will be, provided to me. Thereafter the City of Conover reserves the right to take corrective actions such
as, but not limited to street cleaning at the location of the sediment loss, at that time 1, the undersigned applicant
will be billed for the cost of the street cleaning in accordance with Appendix B. Section 94. of the Conover Code
of Ordinances.
