Facility Located at 1129 Kendall Mill Road, Davidson County
Dear Permittee:
On September 4, 2019 Paul DiMatteo of this office conducted a Compliance Evaluation Inspection on
the subject wastewater treatment facility. The attached inspection form notes the areas that were evaluated for
the inspection, with notable issues summarized as follows:
1. Please ensure that disinfectant is added to keep a continual supply, and that chlorine tablets used for
disinfection are properly labeled for wastewater (generally, tablets that utilize at least 65% calcium
hypochlorite as the primary ingredient, see Part 1, Section D.1(a)).
2. The outlet pipe could not be located. Please note that the Permit requires a visual review of the outfall
location to be executed at least twice per year to verify that there are no visible signs of malfunction (Part
1, Section D.6). Therefore, please ensure that the outfall is located and accessible for inspection.
3. Please be reminded that Part 1, Section C requires the discharged effluent to be analyzed at least once
per year to verify its compliance with effluent limits. The analysis must be conducted by a certified
laboratory. A list of labs in this area is enclosed.

I met Mrs. Suggs during the inspection. She said her septic tank was pumped about a year ago, but
that's all that's been done for upkeep. She knows where the other tanks are but she doesn't maintain
them.

According to records the effluent pipe is actually buried below the ground surface and hasn't
been seen in decades. I tried to go to the area where the pipe is supposed to be but it was
thickly overgrown.

Is the contact chamber free of growth, or sludge buildup?
Is there chlorine residual prior to de-chlorination?
Chlorinator is enclosed in a ~3 foot diameter concrete riser with metal lid on it. There was
vegetation growing on top of the riser lid. It obviously has not been checked in some time.

Effluent sampling is not conducting, and according to permit files there is no discharge to
surface waters because the effluent pipe is buried.
