Here are my additional comments on the SMP for the subject project:
1. The submitted O&M agreement has not been signed/notarized. Also, note that the newer version of the O&M
form is available at https://deg.nc.gov/about/divisions/energy-mineral-and-land-
resources/stormwater/stormwater-program/post-construction.
2. Please complete and provide the most up-to-date Supplemental EZ form for each Bioretention cell. The form is
available at https://deg.nc.gov/about/divisions/energy-mineral-and-land-resources/stormwater/stormwater-
program/post-construction. If you have any trouble using the form, please feel free to contact me.
3. A soil report and/or hydrogeological investigation is needed in order to ensure that the required minimum
separation between the Seasonal High Water Table (SHWT) and the lowest point of the bio cell can be met. In
addition, determination of the in-situ soil infiltration rate is needed for evaluation of the proposed design.
4. Please provide detailed planting plan that meet the minimum design criteria of the Stormwater Design Manual
for Bioretention Cell.
5. The submitted Stormwater Management Report and engineering plans have not been signed/sealed by a NC
licensed professional engineer.
