This form must be completed by Permit Writers for all expedited permits which do not require
full Fact Sheets. Expedited permits are generally simple 100% domestics (e.g., schools, mobile
home parks, etc) that can be administratively renewed with minor changes, but can include
facilities with more complex issues (Special Conditions, 303(d) listed water, toxicity testing,
instream monitoring, compliance concerns).

Facility Overview:
The Neilson WTP operates a conventional WTP designed for a potable flowrate of 48
MGD. The facility currently recycles all backwash and has a monitoring wavier in effect
since 2011. The application states the backwash will continue to be recycled until facility
upgrades are completed (estimated to be 2020 or 2021), which will add an equalization
basin and clarifier to process water for discharge through the permitted outfall and
processing residuals for land application, at which point discharge from the outfall will
commence.
Compliance History:

Ron Boone, of the NC Division of Water Resources (DWR), Winston-Salem Regional Office,
visited the subject water treatment plants on July 18th, 2019, to perform Compliance Evaluation
Inspections (CEI). The assistance and cooperation of Bill Brewer and Billy Goss, Operator in
Responsible Charge (ORC), was greatly appreciated. Details of the inspection are enumerated on the
attached EPA Water Compliance Inspection Report.
Mr. Boone noted no violations or deficiencies at any of the three facilities inspected. However,
you're reminded that failure to comply with all conditions of the subject permits subjects the city to
civil penalties not to exceed $25,000.00 per violation, per day.

The City of Winston-Salem formally requests renewal of NPDES Permit #NC0086011 at our
R.W. Neilson Water Treatment Plant. As requested and required this letter serves as notification
of our intention to renew this permit.
The Neilson Plant is currently operating under a discharge waiver as requested by David
Saunders on October 11, 2011 (letter attached). The City of Winston-Salem has contracted with
Black & Veatch International to design a major upgrade of this facility which is currently at 90%
completion. It is anticipated this project will be bid in March of 2019 and the construction phase
will last approximately 36 months. As part of the design, the facility will have an equalization
basin and clarifier added to process water for discharge through our permitted outfall. Prior to
any discharge my staff will notify the Winston-Salem regional office and upon discharge will
commence sampling for the discharge as stipulated to ensure compliance with the permit.

The source for this facility is a low head dam and intake on the Yadkin River located at Idol's
Dam approximately 4 miles west of the plant.
7. Describe the treatment process(es) for the raw water:
Raw water from the Yadkin River is pumped to two raw water reservoirs (15 MG each for a total
of 30 MG of raw water storage). The raw water then flows by gravity into a 48.0 MG
conventional water treatment plant. Coagulation is achieved using aluminum sulfate followed
by flocculation, sedimentation and filtration. Sodium hypochlorite is added to the settled water
prior to filtration for disinfection. After filtration sodium hypochlorite is again added for
disinfection and to maintain chlorine residual. Zinc orthophosphate, hydrofluorosilicic acid and
caustic soda are added after filtration.

facility:
Three (3) waste station pumps [6.48 MGD each for pumping back wash water to head
of water plant treatment process]
Two (2) lagoon decant pumps [0.58 MGD each for pumping lagoon decant to waste
station]
Three (3) residuals lagoons [-2.1 MG's each]
Neilson's sedimentation basins are currently cleaned manually and the solids are sent straight
to one of three residuals lagoons. Water is decanted off the top of the lagoon in service and is
pumped to the waste station via one of two 0.58 MGD waste pumps. All filter back wash water
at the Neilson Plant is currently recycled back to the head of the water plant treatment process
via a waste pump station that contains three 4,500 MGD pumps. Due to this the NPDES
discharge point is currently inactive. Neilson is plant is currently at 90% design review of an
upgrade that will be bid in March 2019. This upgrade will include the addition of an
equalization basin and clarifier for treating process residuals for land application. When the
upgrade is completed the plant will no longer recycle water but will start to use the permitted
discharge point. This is anticipated to begin sometime in 2020 or 2021.

Solids Handling Plan
The Neilson Water Treatment Plant is a 48 MGD water treatment facility. Solids are
generated by manual sludge removal from sedimentation basins, backwashing filters, and
filter -to -waste after filter backwashes are completed. Solids produced from filter backwashes
and filter -to -waste is routed to a waste pump station and is then pumped to the head of the
water treatment process. Solids removed from the sedimentation basins are routed directly to
one of three residual lagoons.
Permits and Classifications:
1. The discharge from the residuals lagoons is authorized by NPDES Permit NC0086011.
The classification for the discharge is Grade 1 Physical/Chemical.
Operation:
1. The system consists of a waste station with (3) Fairbanks Morse single stage pumps rated
at 6.48 MGD each, (2) Fairbanks Morse basin sludge pumps rated at 1.0 MGD each, (2)
lagoon pumps rated at 0.65 MGD each and (3) residuals lagoons (3 lagoons at —2.0 MG).
Each residuals lagoon consists of an oval earthen lagoon with clay bottom.
2. Routine inspections of equipment will be conducted by Rodney Darr, Sr. Utility Plant
Supervisor, to ensure proper operation of equipment. Any equipment not functioning
properly will be reported through the water division's asset management program,
MAXIMO, to the appropriate support group for repair/replacement.
3. Backwash and filter -to -waste water from the plant process flows to the waste station and
then is pumped by the waste station pumps to raw water reservoir #2 at the head of the
plant treatment process.
4. All sedimentation basin and hopper blow downs are pumped by the basin sludge pumps
directly to one of three residuals lagoons.
5. The solids stream enters on one end of each lagoon and the decant is pumped from the
opposite end by the lagoon pumps to the waste station where it is returned to the head of
the treatment process.
6. Residuals lagoons are rotated with one cleaned and ready for use, one in service and one
drying at all times.
7. Residuals are removed from the lagoons and hauled to an application site which consists
of —31 acres on Cooper Road (about 1.0 mile from the Neilson WTP) by a private
contractor.

8. Once the sludge has dried, it is spread on the application site and a grass cover is planted
to stabilize the area.
Monitoring and Reporting Requirements:
1. Residuals generated by each source -generating facility must be analyzed to demonstrate
that they are non -hazardous. Sampling will be conducted by Rodney Darr, Sr. Water
Treatment Plant Supervisor, once per hauling event as described below.
2. An analysis is required for each residuals land application event.
3. Analyses made according to monitoring and reporting requirements must be certified for
the required parameter(s).
4. Corrosivity, ignitability, reactivity, and a Toxicity Characteristics Procedure analysis are
required with each residuals application event. The TCLP analysis includes the following

7. Proper records are maintained tracking all residuals activities. These records include the
source, volume, analysis, and recipient of the residuals.
8. Three copies, per application event, of all required monitoring and reporting requirements
are submitted annually before March 1, to NC DENR-DWQ.

2. If residuals are sold or given away, one of the Class A pathogen requirements in 15A
NCAC 02T .1106(b) must be met. Aerobic digestion is used to satisfy this requirement.
Application Requirements:
1. Bulk residuals are not applied under the following conditions:
i. If the residuals are likely to adversely affect a threatened or endangered species.
ii. If the application causes prolonged nuisance conditions.
iii. If the land fails to assimilate the bulls residuals or the application causes the
contravention of surface water or groundwater standards.
iv. If the land is flooded, frozen, or snow-covered or is otherwise in a condition such that
runoff of the residuals would occur.
v. During a precipitation event or within 24 hours following a rainfall event of 0.5
inches or greater in a 24-hour period.
vi. Within the 100-year flood elevation, unless the bulk residuals are injected or
incorporated within a 24-hour period following the residuals application event.
vii. During a precipitation event or within 24 hours following a rainfall event of 0.5
inches or greater in a 24-hour period.

viii. If the slope of the land is greater than 10 percent when the bulk residuals are surface

Spills and Noncompliance Notification:
1. The Winston-Salem Regional Office, telephone number (336) 771-5000, will be notified
within 24 hours or the next working day following the occurrence or first knowledge of
the occurrence of any of the following:
i. Any occurrence with the distribution program which results in the land application of
significant amounts of wastes which are abnormal in quantity or characteristic.
ii. Any -failure of the distribution program resulting in a release of materials .to receiving
waters.
iii. Any process unit failure, due to known or unknown reasons, that render the facility
incapable of adequate residual treatment.
2. Persons reporting such occurrences by telephone shall also file a written report in letter
form within (5) days following first knowledge of the occurrence. This report must
outline the actions taken or proposed to be taken to ensure that the problem does not recur.
