Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS: HISTORY 1. The Town of Weldon owns the property. 2. The Solid Waste Section issued a letter on April 19, 1995, stating that the documentation needed for a CCB structural fill site notification had been received and construction could begin. 3. Plumbline Engineers, P.A., 121 Washington Avenue, Weldon, NC 27890, were the engineering firm and contractors for the site. 4. The coal ash came from Roanoke Valley Energy, PO Box 351, Weldon, NC 27890. 5. The Certificate of Closure was dated July 12, 1996. There were 6,689 cubic yards of coal ash deposited at the site. The property was recorded in Deed Book 1677, Page 444 on July 12, 1996. INSPECTION COMMENTS 1. The site is totally grassed and very well maintained. 2.
