Box 159 Merry Hill, North Carolina, 27957 PARTICIPANTS: Daniel Sumerel – General Manager Tom Turri – Project Manager John College – NCDEQ-DWM-SWS-FOB STATUS OF PERMIT: Closed PURPOSE OF SITE VISIT: Partial Facility Compliance Inspection STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 3 ADDITIONAL COMMENTS; 01 - This Facility Compliance Inspection was un-announced due to contact information (Names & Phone numbers) had changed and could not reach old contacts. 02 - It is required, at a minimal to inspect these types of closed facilities at least 1 time per 3 years. 03 - Facility roads are made of all weather material. 04 - There are 8 Groundwater monitoring wells at this facility. Last Groundwater monitoring report dated June 15, 2017. 05 - All 8 wells were locked and found that 5 were ok and 3 wells needed required information to be re- stamped/replaced. Emailed a copy of the March 23, 2017 Well ID memo to Daniel. 06 - No Edge of Waste Markers were found. Emailed a copy of the Waste Marker memo dated May 29,2009 to Daniel. 07 - There are several structures constructed on top of this landfill. 08 - The landfill appears to be well maintained, there were no cover/erosion issues observed. There was snow covered areas today during the inspection. 09 - Corrective actions need to be taken to # 5 and #6 of this report. 10 - Digital pictures taken during this inspection and attached to this report.
