Register of Deeds office on October09, 1998, Deed Book 1638, pages 182 – 193. Doc. ID 16129. Expires – 07-01-2018. PURPOSE OF SITE VISIT: Partial Facility Compliance Inspection STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 3 ADDITIONAL COMMENTS; 1 - This facility only receives Solid Waste from Nash County. 2 - The CDLF is being covered daily with adequate cover. No unpermitted Solid Waste noted during this inspection. 3 - Access road is made of all-weather material and in good shape. 4 - Signs have all required information. 5 - Scrap tires are kept inside a closed trailer. 6 - Yard Waste area looked OK. Need to remove plastic bags from waste stream. Better screening at the scale house needed on the incoming yard waste. 7 - Records were reviewed during this facility inspection and found to be in order. 8 - No eroded areas noted on the CDLF. Side slopes looked good. 9 - Certified Landfill Operation Specialist: Pamella L. Brinkley – expires 10/03/2020, Sadie H. Tucker – expires 05/19/18, Gregory Torey – expires 05/09/2019 and Emma Brantley – expires 05/24/19. 10 - White goods /scrap metal area is looking good. 11 - Facility is currently working on getting all wells identification tags replaced where needed. All wells looked at had locks. 12 - John College and Ben Barnes screening 1 load of CD waste from a Waste Industries truck while conducting this facility inspection. No un-permitted waste found. 13 – Corrective Action needed on the Yard Waste Area. 14 – Digital pictures taken during this inspection.
