CDLF Susan Heim, Environmental Senior Specialist – Solid Waste Section STATUS OF PERMIT: Permit to Construct and Operate Phase VI and Vertical Expansion over Phases IV-VI issued April 6, 2015 Permit to Operate Expires: April 6, 2020 (submit renewal request by October 6, 2019) PURPOSE OF SITE VISIT: Comprehensive Inspection STATUS OF PAST NOTED VIOLATIONS: N/A OBSERVED VIOLATIONS: None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 3 ADDITIONAL COMMENTS 1. The facility operates as a construction and demolition landfill with a treatment and processing area for concrete, asphalt and brick (CAB). 2. The facility is permitted to receive C&D waste, inert debris, land clearing waste and asphalt generated within Forsyth County and its municipalities. 3. Proper signage was observed at the entrance, and informational signs were posted throughout the facility to direct and assist the public. 4. The current Permit to Operate and Operation Plan were available and reviewed. 5. Waste screening logs for the period January 1, 2017 through October 12, 2017 were reviewed. Logs that documented unacceptable wastes found in the screened loads also indicated the disposition of the unacceptable waste. Screening was conducted at the rate of 6 per day, which meets the standard set in the approved Operations Plan of screening 10% of all incoming loads. Mr. Fultz estimated between 50 and 100 loads are received at the facility daily. It is recommended that the number of screenings be increased on days when incoming traffic is heavy. 6. Review of methane monitoring records demonstrated that methane monitoring is performed quarterly as required. Records for 12/20/16, 3/31/17, 6/28/17 and 9/25/17 were verified and reviewed. Facility staff also screen for hydrogen sulfide gas during these monitoring events. 7. Groundwater monitoring and leachate sampling have been taking place on the required semi-annual basis, as indicated by the reports on file. Records for the first semi-annual sampling event for 2017, performed May 30 through June 1, 2017, were reviewed and verified. A total of 4 surface water points and 14 monitoring wells were sampled. Field sampling notes showed no issues with any of the monitoring wells themselves. 8. Training and certification records indicated the following: • Adam Rickett, Manager of Landfill Operations Certification No. 92044, expires 05/06/2020; • Gray Fultz, Landfill Operations Specialist Certification No. LF-2008011, expires 02/08/2020; • John D. McClintock, Landfill Operations Specialist Certification No. LF-2015018, expires 03/24/2018. 9. Tonnage records for the period January 1, 2017 through September 30, 2017 were reviewed. During fiscal year 2016-17, a total of 42,426.25 tons of C&D waste was accepted at the facility. This total equates to an average of approximately 3,536 tons of waste per month. C&D Landfill Operation: 10. At the time of this inspection, waste was being placed in Phase IV of the facility’s vertical expansion. 11. Edge of liner markers were present and easily spotted during the inspection. 12. Roadways were of all-weather construction and very well maintained. 13. The working face was of a manageable size and sufficiently flat to enable the equipment operators to effectively control and compact the waste. Mr. Fultz explained that he had designed two tipping areas. The larger, which was in use at the time of the inspection, is used during dry weather. It has a wider tipping area that can accommodate a number of trucks at one time. Mr. Fultz pointed out a smaller area south of the current working face that he said he uses during wet weather. The smaller working face and tipping area make this spot easier to maintain during wet weather. 14. No unacceptable waste was observed at the working face. 15. Operational cover was very good. 16. Side slopes of the landfill were well maintained and in good condition. Vegetative cover was mown and no woody vegetation was observed on the landfill cap. 17. No evidence of erosion was noted during this inspection. 18. Several groundwater and methane monitoring wells were observed during the inspection. All were easily accessible. 19. The facility accepts loads of concrete, used asphalt and brick that are directed to a separate collection area located within sight of the scale house. Each load is screened by the scale house operator and, if found to contain acceptable materials, directed to the CAB area to be unloaded. Several times per day, the material in the CAB area is pushed up by facility staff, who also remove any unacceptable material found in the pile. During FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 3 of 3 the inspection, several pieces of painted concrete were observed. Mr. Rickett stated that they would be removed and disposed of in the working face before the end of the operating day. 20. CAB materials are sorted and stored onsite prior to processing. Mr. Rickett stated that crushing occurs approximately once every 18 months. Processed materials are then stored in an area north of the CAB area for use as needed at the site. 21. The CAB area was marked with informational signage and appeared to be clean and well-maintained. 22. Windblown litter was well controlled around the working face and throughout the facility. During the inspection, facility staff were observed picking up windblown litter, and Mr. Rickett stated that this task occurs daily. 23. The facility appeared to be clean and well maintained. 24. The new temporary disaster debris storage site (DS34-006) was observed during the inspection. The 12.74-acre area, located southeast of the active landfill, was unmown, and the access roadway stopped a few yards into the site. Mr. Rickett stated that bushhogging and extending the access roadway would be required prior to activating the site.
