The permit shall expire July 30, 2018. PURPOSE OF SITE VISIT: Comprehensive Inspection STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. Pursuant to 15A NCAC 13B .0201(g), no later than January 30, 2018, the owner or operator must submit a request to the Section for permit review prepared in accordance with Section .1603(a)(2) or (3) as applicable, and must update pertinent facility plans including, but not limited to, the facility operation and waste screening plans. 2. Proper signage was observed at the entrance of the facility. 3. The facility is permitted to receive solid waste generated within Iredell County. 4. The facility accepts an average 684.31 tons per day. Operations Inspection of the MSW Landfill Unit: 5. The facility was currently working in Phase 5. 6. The landfill was actively receiving waste during the inspection. Four tires were removed from the working face to be disposed at the tire collection area. No other unacceptable waste was observed. 7. The working face was small and compact. Facility staff was applying soil cover during the inspection. Facility staff applying soil cover. Facility utilizes soil and enviro cover. Windblown waste was being collected during the inspection. Operations Inspection of the Closed C&D Landfill Unit: 11. The CDLF discontinued acceptance of waste on June 27 2008. 12. The post closure period for the CDLF began on May 8, 2013. 13. A good stand of vegetation was observed on the CDLF. 14. Edge of liner markers were observed. Operations inspection of the Bailer Facility: 15. The Iredell County Baler Facility (Permit # 49-03-TP) has been incorporated into the Iredell County MSWLF Permit # 49-03. 16. The baler facility is used by the general public and small generators as a drop off area. Waste is dumped onto the tipping floor and loaded into roll-off boxes to be hauled to the MSWLF. 17. Only recyclable material is baled at the facility. 18. Periodic random inspections are conducted inside the baler facility. Operations Inspection of the White Goods Collection Area: 19. This area was well managed at the time of inspection. 20. White goods containing Freon are separated from other metals and stored in an upright manner. 21. Freon is extracted by Landfill staff. This area was well managed at the time of the inspection. 23. Latex paint collected in five (5) gallon containers is mixed with the Posi-Shell. Operations Inspection of the Tire Collection Area: 24. This area was well managed at the time of inspection. 25. All tires were being collected in covered New River trailers and were being stored properly. 26. A New River employee is on site Monday through Friday 8 am – 3 pm. Customers unload the tires onto the loading dock and the New River employee laces the tires in the trailers. Tire collection area. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 5 of 5 Operation Inspection of Mulching and Grinding / Yard Waste / Type I Small Composting Area: 27. This area was well managed at the time of the inspection. 28. Land clearing waste and pallets are kept separated from yard waste that is required to be composted. 29. Several pallets were set aside including the painted pallets to be used in the electronic collection area. Operations Inspection of the Asphalt Shingle Recycling Area: 30. The asphalt shingle recycling area was well managed at the time of the inspection. 31. No unacceptable waste was observed. Records Review: 32. Reviewed permit, operations plan, training records, semi-annual groundwater, surface water and leachate monitoring reports (January 2017), quarterly methane monitoring reports (February 2017 & August 2017), random inspection records (2-3 per week), waste determination records, leachate lagoon inspections, leachate discharge records (approximately 8000 gallons discharged weekly), scrap tire certification records (178,893 tires disposed 2016-2017) and shingle recycling records (tonnage and asbestos sampling). 33. Yard waste compost temperature records were reviewed and are being maintained in accordance with the rules. 34. The leachate lines are flushed once per year. The leachate lines are videoed every five (5) years. Leachate lines were videoed August 2017 by Jetclean America. 35. Certified Operators are as followed: Terence Boller – Certified Manager of Landfill Operations expires June 6, 2020. Larry E. Beam, Jr. – Certified Manager of Landfill Operations expires June 6, 2020. David Lambert – Certified Manager of Landfill Operations expires May 7, 2019. Jamie Buzze – Certified Landfill Operator expires January 25, 2020. Certified Technical Associate of Landfill Operations expires June 15, 2020. Rhyne Anderson – Certified Landfill Operator expires January 25, 2020. Wesley Femister – Certified Landfill Operations Specialist expires March 12, 2019. Jeffrey A. Freeze – Certified Landfill Operations Specialist expires March 12, 2019. Timothy H. Marlow – Certified Landfill Operations Specialist expires September 12, 2019. Sherry H.
