Life of Site permit issued on December 29, 2016. PURPOSE OF SITE VISIT: Partial Facility Compliance Inspection. STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS; 1 – This facility is adequately secured. 2 – This facility is permitted to receive solid waste generated in Pasquotank and Camden Counties at 20,000 tons per year. 3 – Signs posted at the entrance have all required information as stated in the permit. 4 – Interior roadway is constructed of all-weather material and in good condition. 5 – This facility has several Certified Landfill Operations Specialist: Partial listings of operators: Justin Seth Stevenson, expires 02/26/19. Carl Douglas Swain, Jr, Expires 12/06/18. Hunter E. Stevenson, expires 12/10/18. Melvin Owens, expires 08/27/18. Has more operators not listed on this inspection report. 6 – This facility also has a Certified Manager of Landfill Operations. Brad Gardner, issued on 06/15/17 and expires on 06/15/20. Certification number: 56116. 7 – Permit, approved plans and required were available for review. 8 – Inspections records were reviewed at the C&DLF office. All incoming loads are weighed and has the origins of loads on the load tickets. 9 – Edge of Waste markers easily identified. 10 – Site has plenty of cover material available. 11 – Working face is less than a quarter acre and is being covered at least weekly and more frequently if needed. 12 – The used tire storage area appears to be managed well. 13 – The LCID Storage and Grinding area approximately 1.5 acres. No more than 4,000 cubic yards of material should be stored on-site at any time per your permit. Site has less than 4,000 cubic yards during this facility inspection. Please be sure to remove any plastic bags and manufactured wood from this area before grinding. 14 – White Goods and Scrap Metal is being managed well. 15 – The recycle area is being managed well. 16 – Digital pictures taken during this partial facility compliance inspection. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
