Dear Ms. McHargue:

The Division of Waste Management (DWM), Solid Waste Section (the SWS) received and
reviewed the flowing documents:
• Amendment of Permit 34-02, Hanes Mill Road Landfill, Winston-Salem, North Carolina.
Prepared by HDR Engineering, Inc. of North Carolina, Charlotte, NC (HDR). June 3, 2016 and
revised through July 07, 2017. (DIN 28143).
• As-Built Certification Documentation for the Hanes Landfill Cell 5 Expansion. Prepared
by HDR. January 2017 and revised through April 28, 2017. (DIN 27721).
Based on the completed reviews, the SWS approves the above-referenced documents and issues
the City of the Winston-Salem (the Owner) and the City of the Winston-Salem/Forsyth County
City/County Utility Commission (the Operator) a life-of-site Permit Approval to Operate for
operating Hanes Mill Road Municipal Solid Waste Landfill (MSWLF) – Phases 1 & 2 (Cells 1
through 5), Permit No.3402-MSWLF-1997 and the Permit for Closure for the closed Hanes Mill
Road Sanitary Landfill, Permit No. 34-02.

According to North Carolina General Statue (N.C.G.S.) 130A-294(a2), this permit for the Hanes
Mill Road MSWLF – Phases 1 & 2 (Cells 1 through 5), Permit No.3402-MSWLF-1997 shall
expire on March 06, 2030, which is consistent with the term of the thirty-(30)-year Franchise
Agreement (DIN 25851). The permittee may request an approval from the Section to adjust the
permit expiration date to such time a new Franchise is obtained by the permit holders at which
time the term of this permit will be adjusted accordingly.

Enclosed please find a hard copy of the permit (DIN 27884) and the certified copy of the permit
for the permit recordation purpose. The approved permit application documents DIN 28143 can
be downloaded from the following web link
http://edocs.deq.nc.gov/WasteManagement/Search.aspx. Please place the permit and other
approval documents to the facility operating record and also carefully review the enclosed permit
and pay special attention to the following permit condition, which shall be completed
accordingly. The permit conditions briefly stated below:
• General Permit Condition No. 3, Part I, Attachment 1 of the permit regarding permit
recordation requirement. The certified copy of the permit (enclosed) must be filed in the
Register of Deeds office in Forsyth County, and the copy (entire document including
permit) is returned to the SWS, within thirty (30) working days, from the date received,
with the page and book number, date of recordation, and Register’s seal.
