Dear Mr. Price:
On June 1, 2017 the Division of Waste Management, Solid Waste Section (Section) received Charah, Inc.’s construction
quality assurance (CQA) documentation for Subcell 2F of Permit No. 1910-STRUC-2015, the Brickhaven No.2 Mine
Tract “A” Mine Structural Fill (Facility). Following comments from NCDEQ on June 15, 2015, the below entitled
document was revised on June 15, 2017
Construction Certification Report, Brickhaven No.2 Mine Tract “A” Structural Fill – Cell 2, Subcell 2F, Permit 1910-
STRUCT-2015, Charah, Inc., Moncure, North Carolina, May 31, 2017 Revised June 15, 2017. Prepared for Charah.
Prepared by HDR Engineering. May 2017. DIN 27977.
The Section has reviewed the documentation submitted on behalf of Charah, Inc. and hereby approves the
commencement of structural fill operations in Subcell 2F at the Facility. Please note the following requirements for future
construction and operation of the Facility;
 Document ID Number 27977 is considered an approved document of the Facility’s permit and will be added to
the list of approved document within the permit the next time the Facility’s permit is either amended or modified.
 Charah, Inc. must submit construction quality assurance documentation for all future construction and receive
approval from the Section prior to placing those areas into operation.
