OBSERVED VIOLATIONS:
1. 15A NCAC 13B .0566 (1): Operational plans shall be approved and followed as specified for the facility.
2. 15A NCAC 13B .0566 (4): Adequate soil cover shall be applied monthly or when the active area reaches one
acre in size, whichever occurs first.
3. 15A NCAC 13B .0566 (3)(j): Excavation, grading and fill material side slopes shall not exceed three to one
(3:1).

ADDITIONAL COMMENTS
1. Facility permit expired on January 20, 2017. Permit renewal period extension was requested on behalf of
Autry Grading by Joyce Engineering and approved on March 30, 2017 by Elizabeth Werner, SWS
Permitting Hydrogeologist. Permit renewal shall be submitted to the Section by July 1, 2017.
2. Please remember unacceptable waste must be removed as it is observed and disposed of properly
3. The sign at the entrance sign at the entrance to the facility was correct.
4. Site had been regraded since the previous inspection on January 19, 2017. There was no ponding water over
waste during this inspection.
5. Continue to work on covering all exposed waste. Waste must be covered and provisions for a ground
cover sufficient to restrain erosion accomplished within 30 working days.
6. Sedimentation basin had been cleaned and regraded since the previous inspection.
7. Continue to work on covering and grading the vertical side slopes. Exposed waste was still observed
during this inspection. Side slopes should not exceed a 3:1 ratio during filling or covering operations.
8. Required per the approved plan is a 25-foot gravel access road around the entire perimeter of the
landfill. Access road should be of all-weather construction and properly maintained.
9. 120 calendar days after completion of any phase of disposal operations, or upon revocation of a permit, the
disposal area shall be covered with a minimum of one foot of suitable soil cover sloped to allow surface water
runoff in a controlled manner.
10. Corrective measures are necessary to bring the landfill into compliance and should be completed within
30 days’ receipt of this inspection report.

administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
