Subject: Brickhaven No. 2 Mine Tract “A” Site (NCDEQ Landfill Permit 1910-STRUCT-2015) –
Monitoring Well Installation and Piezometer Abandonment

Dear Ms. Werner,
HDR Engineering, Inc. of the Carolinas (HDR), on behalf of Charah, LLC (Charah), was tasked with
the installation of one (1) replacement groundwater monitoring well and abandonment of five (5)
piezometers at the Charah Brickhaven No. 2 Mine Tract Site “A” (Site) in Moncure, Chatham
County, North Carolina. This work was performed following the completion of initial permitting and
operational activities. HDR is hereby submitting this summary of events to resolve the matter
concerning groundwater monitoring well installation and piezometer abandonment at the Site.
Monitoring Well Installation 
On April 20, 2017, HDR provided oversight for the installation and development of a replacement
monitoring well at the Site. Michael Wilson (NC LIC#3351A) with South Atlantic Environmental
Drilling and Construction Company, Inc. (SAEDACCO) of Fort Mill, South Carolina installed MW-7R
to a total depth of 36 feet below ground surface (ft.-bgs) with a screened interval of 26 to 36 ft.-bgs
(Attachment 1). The well was completed to NCDEQ monitoring well standards including a 2’x2’
protective well pad and stainless steel stick up casing and proper well ID. MW-7R was a
replacement for monitoring well MW-7 which had intermittently insufficient water column for
sampling likely due to the installation of a retaining wall in close proximity to the well location. The
monitoring well was re-located approximately 125 feet to the southeast of its initial location.
Attachment 2 depicts the North Carolina Department of Environmental Quality (NCDEQ) Well
Construction Form for MW-7R installation activities. Attachment 3 includes the new survey
location as well as Top of Casing (TOC) and ground surface elevations. MW-7 will temporarily
remain onsite for groundwater gauging only until construction activities are complete and safe
access to the existing location is merited.
Monitoring Well Abandonment 
On April 18 through April 20, 2017, Michael Wilson with SAEDACCO abandoned five (5)
piezometers including PZM-3, PZM-4, PZM-6, PZM-8, and PZM-19. Previously discussed closure
requirements (i.e. over-drilling) with the NCDEQ, Division of Waste Management (DWW) – Solid
Waste Section were utilized. That abandonment work is detailed in the Attachment 4 table.
Additionally, Attachment 5 depicts the NCDEQ Abandonment Record for those aforementioned
piezometers. Attachment 6 also includes a photographic log for all installation and abandonment
related activities at the Site.
