STATUS OF PERMIT: Original Permit to Construct (PTC)/Permit To Operate (PTO) Phase I issued February 19, 2010 Administrative Correction of Permit Number March 15, 2010 PTC Cell 2/PTO Cell 1 and 2 issued June 2, 2015 PTC expires September 17, 2020 PTO expires September 17, 2020 PURPOSE OF SITE VISIT: Comprehensive Inspection STATUS OF PAST NOTED VIOLATIONS: NONE OBSERVED VIOLATIONS: NONE FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS On January 21, 2016, Liz Patterson and Drew Hammonds met with Jeff Lees to conduct a comprehensive inspection of the Weeks Sand Pit #2 LCID Landfill located at 20796 N.C. Highway 24-27 in Harnett County. 1. The facility is a land clearing and inert debris (LCID) landfill. 2. The facility has proper signage. 3. The PTO expiration date is September 17, 2020. The permit renewal application due date is March 17, 2019. 4. The facility is properly secured with a locked gate. Personnel are on site during operating hours. 5. The facility is permitted to receive waste generated within Harnett County, Wake County, Johnston County, Cumberland County, Sampson County, Moore County, and Lee County. 6. The facility is permitted to receive land clearing waste, yard trash, untreated and unpainted wood, uncontaminated soil, inert debris, and asphalt. 7. Emptying out concrete mixing trucks within the permitted landfill is not approved and must stop. 8. The facility site plan, operations plan, and permit are kept on site and were reviewed with the site operator. 9. The landfill working face was less than one acre and is being managed properly at this time. 10. Excavation, grading and fill material side slopes shall not exceed three to one (3:1). 11. Stockpiled materials must be moved to areas designated on map in Cell 2 per approved plan and map. 12. The facility must be operated in accordance with the approved operations plan and permit. Changes or modifications to the design or operations of the facility including storage areas must be submitted to the permitting branch for approval before making changes. An updated will be required to show changes. 13. Construction of Cell 2 must be completed in accordance to the approved plans submitted to the division. All necessary documentation and certifications related to the construction and preparation of Cell 2 must be submitted to the permitting branch as required before commencing operations. 14. Trucks and equipment should not be parked on top of landfill. 15. Access roads are of all-weather construction. 16. Digital pictures were taken during the inspection. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
