Permit renewal in process. PURPOSE OF SITE VISIT: Comprehensive Facility Compliance Inspection STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS: 1 – Permit expired November 17, 2015. Being review by Pat Backus – DWM permitting. 2 – This facility is permitted to receive Municipal Solid Waste as defined in N.C.G.S. 130A – 290 (a) (18a). 3 – Random inspections are conducted at the scales and on the tipping floor. Inspections are recorded. 4 - This facility is secured by fences with locking gates. 5 - Signs are posted on the fence before the scales with all required information. 6 - Interior roadways are made of all-weather construction and maintained well. 7 - This facility has 4 Certified Transfer Station Operations Specialist. Operator Certifications include: Jeffrey L. Morgan, expires 10/2019; Oral Pittman, expires 11/2017; David Styles expires 12/2017, Jacob R. Parker expires 5/2017, 8 - Copies of the permit and required records were available for review and were kept in good order. 9 – White goods area looked good. 10 – Tipping floor is kept clean. 11 – Windblown trash was not an issue during this facility inspection. 12 – Site has 6402T-TRANSFER-1997 on site also. 13 – Overall facility is being managed well. 14 – Digital Pictures taken.
