STATUS OF PERMIT:
Used as recycle area as stated in 6405T-Transfer-2001, Life of Site permit issued on April 10, 2017. Attachment
3, part 2 (3). If want to start back using this facility as MSW Transfer Station, DWM Must be Notified and
facility inspected prior to accepting solid waste.

PURPOSE OF SITE VISIT
Comprehensive Facility Compliance Inspection

STATUS OF PAST NOTED VIOLATIONS:

OBSERVED VIOLATIONS:
None

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS:
1 - The facility is permitted to receive 350 TPD.
2 - The hours of operation are M-F 7:00AM to 4:00PM.
3 - Current use for the facility is as a collection and transfer point for recycled materials. If the facility is to resume
accepting MSW the Division must be notified and an inspection conducted.
4 - Signage meets required language in the permit.
5 - Fire protection is by a 1inch water hose. Two fire extinguishers were available for use.
6 - The cleanliness of floors walls and ceilings was acceptable. Insulation on the ceiling is peeling.
7 - Blown paper was well controlled.
8 - White goods and electronics are being collected at the facility.
9 - The area beneath the compactor has the normal amount of waste to be expected during daily operations.
10-Operator Certifications include: Jeffrey L. Morgan, expires 10/2019; Oral Pittman, expires 11/2017; David Styles
expires 12/2017, Jacob R. Parker expires 5/2017, Mr. Parker has attended class for CEC’s.
11-Records were provided and in good order.
12-This facility is very well maintained at the time of this Inspection.
