An extension request must be submitted to the Section 90 days prior to this expiration date. For information concerning this Notification extension, please contact Tony Gallagher; Composting & Land Application Branch Head at 919.707.8280 or via e-mail at: tony.gallagher@ncdenr.gov PURPOSE OF SITE VISIT: Comprehensive Facility Compliance Inspection STATUS OF PAST NOTED VIOLATIONS: None applicable OBSERVED VIOLATIONS: No violations were observed during this Facility Compliance Inspection; please see the ADDITIONAL COMMENTS section below. ADDITIONAL COMMENTS: 1) This Facility is a Wood Ash Storage Facility approved for the storage of wood ash derived from the burning of bark and wood residuals at the Geaorgia-Pacific Wood Products Plant in Dudley, Wayne County, NC. 2) Ash brought to this site for storage must be tested annually in accordance with the approved Craven Ag Services Ash Management Plan. Analytical results from sampling conducted by Pace Analytical Services, Inc of Huntersville, NC have been submitted to the Section for review. 3) This site is secured by a locked chain link gate at the Facility entrance and has controlled access for safety. 4) The Facility access road is of all-weather construction and is well maintained at this time. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section LASA9601_INSP_20170330 Page 2 of 3 5) Ash brought to the site may be stored for 6 months before re-use. 6) The site has been graded to prevent stormwater from running onto the storage area. No ponding water was observed during this Facility Compliance Inspection. 7) Wood ash has been brought on-site and staged for re-use as an agricultural soil amendment. 8) The storage piles are being maintained in a manageable size. 9) Access around the Facility is stable and no areas of erosion were noted during this Facility Compliance Inspection. 10) All buffers and setbacks are being maintained as required. 11) No off-site erosion was observed during this Facility Compliance Inspection. 12) No violations were observed during this Facility Compliance Inspection.
