The Great Oak Landfill is a municipal solid waste (MSW) landfill owned by Randolph County and operated by Waste Management of Carolinas, Inc. 2. This is a new landfill. The facility was issued a permit to construct by the SWS on January 19, 2016. 3. The PTC was recorded with the Randolph County Register of Deeds on February 1, 2016, in deed book: 2479 at pages: 74-93. 4. The facility pre-construction meeting was held on May 24, 2016. 5. The initial cell of the first phase of the landfill has been constructed. The facility pre-operational inspection was conducted on December 20, 2016. 6. The facility shall obtain a permit to operate issued by the SWS prior to receiving solid waste. 7. The facility has constructed Phase 1, Cell 1A. Solid waste will be placed in Cell 1A when it begins operation. 8. The facility has begun construction of Phase 1, Cell 1B. 9. The facility will operate Monday through Friday 7:30 am to 5:00 pm and Saturday 7:30 am to 12:00 pm. 10. The facility permit, site plan, and operations plan were discussed. Copies of pertinent documents are maintained, including the franchise agreement. 11. The facility is permitted to receive solid waste from within the State of North Carolina. 12. The facility shall have a certified operator/operators on site while the landfill is open to the public. Mr. Moore stated that a certified operator/operators will be on staff by the time the facility begins operation. 13. The facility will conduct waste screenings of at least 1 percent of commercial loads per year. 14. The facility does not accept scrap tires. 15. The facility does not accept white goods. 16. The facility has inbound and outbound scales. Each has an outside weight display unit. 17. A vehicle scale test was conducted by J.A. King & Company, LLC (inbound and outbound) on December 5, 2016. A finding of zero error is indicated for each scale. 18. Mr. Moore stated that the North Carolina Department of Agriculture & Consumer Services has been informed that the facility has installed vehicle scales. 19. The facility is permitted to co-dispose of wastewater treatment sludge. 20. The facility is permitted to dispose of shredded tires. Tires shall not be shredded at the facility. 21. The facility is permitted to use alternative daily cover (ADC) that has been previously approved at another sanitary landfill in North Carolina. The SWS maintains a list of approved ADC and its appropriate use. 22. The operations plan states that the facility may use reusable tarps, auto-shredder fluff, or petroleum contaminated soil as ADC. 23. The facility shall maintain records for solid waste used as ADC. 24. Regulated asbestos-containing material shall be managed in accordance with 40 CFR Part 61, Subpart M and 15A NCAC 13B .1626(1)(d). 25. The facility has six groundwater monitoring wells: MW-1, MW-2, MW-3, MW-4, MW-5, and MW-6. 26. During the inspection five groundwater monitoring wells were observed: MW-1, MW-3, MW-4, MW-5, and MW-6. 27. Groundwater monitoring wells MW-4, MW-5, and MW-6 should be relabeled accordingly. 28. Ensure stormwater run-on is diverted from groundwater monitoring well MW-4. 29. The facility has two surface water monitoring locations: SW-1 and SW-2. The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 3 of 5 30. The facility will conduct semi-annual groundwater and surface water monitoring. The sampling events will likely by conducted by Golder Associates, analyzed by Environmental Conservation Laboratories, Inc. (ENCO). 31. The facility has four landfill gas wells: MMW-1, MMW-2, MMW-3, and MMW-4. 32. During the inspection the four landfill gas wells were observed. 33. The facility will conduct quarterly landfill gas well monitoring for methane. The sampling events will likely be conducted by Golder Associates. 34. Ensure stopcock valves or quick release connectors are installed on the landfill gas wells prior to the commencement of quarterly monitoring. 35. A readily accessible unobstructed path to groundwater wells, surface water locations, and landfill gas wells should be maintained. 36. The facility has a 250,000-gallon leachate storage tank. During the inspection the level of liquid in the tank was 29 feet. 37. The leachate storage tank is connected to the City of Asheboro sanitary sewer. 38. The facility has an Industrial User Pretreatment Permit (IUP) No. 4953 issued by the City of Asheboro, effective December 9, 2016 - expiration December 9, 2021. 39. The leachate collection lines of the lined landfill are required to be camera inspected upon completion and at least once every five years. The leachate collection lines were camera inspected by Florida Jetclean on November 8, 2016. Documentation was submitted to the SWS Permitting Branch. 40. The facility shall conduct weekly inspection of the leachate storage tank overfill control equipment. 41. The facility shall conduct weekly inspection of the exterior of the leachate storage tank. Annual internal inspection of the leachate storage tank is required as specified in the facility operations plan. 42. The leachate sample collection point (at the leachate storage tank) should be labeled accordingly. It is suggested that an elbow pipe be installed on the sample port. If a secondary leachate sample point could be used it should also be labeled accordingly. 43. The facility will conduct semi-annual leachate sampling concurrently with the groundwater and surface water monitoring events. 44. Phase 1, Cell 1A has leachate sump digital gauges located adjacent to the side slope risers for the leachate collection system (LCS) and the leak detection system (LDS). 45. The facility shall calculate the action leakage rate (ALR) associated with the LDS. The facility will convert the weekly LDS flow rate to an average daily flow rate in gallons/acre/day for each sump. 46. The ALR is 182 gallons/acre/day. If the ALR is exceeded, landfill staff will implement the Response Action Plan as specified in the facility operations plan. 47. The facility shall conduct weekly inspection of above ground portions of the leachate force main. 48. The facility shall conduct weekly inspection of the underground portion of the leachate force main at dedicated manholes. Inspection of the manhole(s), located at low points along the force main, is by visual alarm mounted atop each junction manhole. 49. The facility was issued a Division of Air Quality (DAQ) Title V Permit No. 10446T01, effective March 17, 2016 – expiration February 28, 2021. 50. The facility has a National Pollutant Discharge Elimination System (NPDES) permit General Permit No. NCG120000, Certificate of Coverage No. NCG120107. The NPDES permit was issued on October 27, 2016. 51. The facility has an erosion and sedimentation control plan RANDO-2016-010 for Phase 1, Cells 1A and 1B. The erosion and sedimentation control plan was approved on May 4, 2016. 52. Erosion, drainage, and sedimentation controls observed appeared to be in good order. 53. Sediment basins are required to be cleaned out when sediment accumulates to ½ of the design depth as specified in the facility operations plan. 54. The facility has installed edge of waste markers (4x4 wooden posts w/white lettering). 55. Windblown material shall be collected at the conclusion of each day of operation. 56. The facility has a soil stockpile area on site. 57. It was stated that the facility will have a water truck available for dust control. 58. The facility will install a permanent truck wheel wash. 59. The facility financial assurance is up to date. 60. The Randolph County Fire Department can be contacted to respond to an emergency at the facility. Mr. Moore was asked to add facility contact information. 63. The facility entrance is secured by a metal gate.
