Scheer, P.E., Vice President, Senior Engineer – Smith + Gardner STATUS OF PERMIT: Permit to Operate issued September 30, 2015 Permit expires March 14, 2019 PURPOSE OF SITE VISIT: Site Inspection and pre-operative meeting STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 3 ADDITIONAL COMMENTS On November 22, 2016, Liz Patterson, Ming Tai Chao, and Shannon Aufman met with Amanda Bader, PE, Andrew Holland, Randy Baker, Larry Thomas, David Baker, and Pieter K. Scheer, P.E to conduct a site inspection and pre- operative meeting of the Harnett County Anderson Creek C&D Landfill located at 1086 Poplar Drive Spring Lake, NC 28390 in Harnett County to satisfy the requirements of Permit condition Attachment 2, Part I, Item 1. 1. The facility is fenced and is secured by a locking gate when it is not open for operation. 2. The correct signage is at the entrance of the facility listing the facility name, permit number and emergency contact information as well as materials accepted at the facility. 3. The facility accepts waste Monday through Saturday from 7:30 am until 4:30 pm with the exception of Wednesday 7:30 am until 12:00 pm. 4. Facility permit, operations plan, and records were not reviewed at this inspection. 5. The permit to operate expires March 14, 2019. 6. The facility is permitted to receive C&D waste generated within Harnett County. 7. The facility is approved to accept the waste disposal rate up to 35,000 tons per year, average of 125 tons per day based on 280 operating days per year, consistent with the resolution passed by the Harnett County Commissioners on December 5, 2009 8. Construction of the remaining area of Phase III - Cell 2 has been completed in accordance to the approved plans submitted to the division. Remaining area of Phase III – Cell 2 facing west The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 3 of 3 9. The working face was being well managed and was not exceeding the ½ acre size limit. Six inches of earthen material should be properly applied once weekly or when the working face reaches ½ acre. Working face of landfill 10. The waste type was construction and demolition waste. No unacceptable waste streams were noted. 11. Windblown waste is being well-managed, collected and returned to the working face prior to close of business daily. 12. The erosion and sedimentation control measures were reviewed. No off-site sediment was noted. 13. The edge of waste markers are installed and being maintained around the entire landfill. 14.
