Appendix H – Soli Liner CQA Data doesn’t include all appendices and the title of each appendix is incorrect. 2. Appendix K3 CQA-Test Results – Protective Cover is not included in the draft CQA Report. (Section 3.0 Summary of Construction Activities) Please provide the following information: 3. In each subsection please provide the in-place quantity of each component - compacted clay liner (CY), textured and smooth HDPE liners (SY or SF), single and double bounded drainage composites (SY or SF), protective layer (CY) of the landfill base construction. The info that can be placed in a summary table will be used to verify if the test number meet the specifications or CQA Plan. Appendix C - Well Abandonment Info 4. The monitoring wells, OWB-16, OWB-16A & OWB-50 are installed inside Cell 9 as show on the Sheet No. 3/Drawing No. S2 of the approved PTC Application (DIN 23822). Please provide the status of the well. Should these wells be plugged and abandoned [P&A] prior to Cell 9 construction, please provide the copy of the abandonment logs. 5. (The 3rd Paragraph in the June 22, 2015 letter) The well OWB-59 is likely a typo. 6. (Figure 2) The gas well GM-22 is proposed to P&A, but no data is available? Please provide the status of the well. Appendix H 1 7. (CQA Test Results – Borrow Material Control Tests, Summary Table) Please make necessary corrections on the summary tables: i. The test results of soil classification for Test Number CCL-22 are incorrect; they are duplicate of those for Test Number CCL-23. ii. There are two compaction test results for Test Number CCL-06 and one of them is incorrect. Please make correction. iii. The results of compaction effort and hydraulic conductivity are not available for the soil sample CCL-18. iv. Additionally, the lab test report of the hydraulic conductivity test for the soil sample CCL-18 are not included in Appendix H1. Please provide data and revise summary table. v. The compaction effort for the following samples are incorrect – CCL-07 through CCL-24. vi. The incorrect data of the maximum density and optimum moisture content for the samples are CCL-07 through CCL-24 is present in the summary table. Appendix H 2 8. (Grain Size Analysis Summary Table) The lab reports contain the moisture content result for each soil sample, please add the moisture content results to the summary table. 9. (Moisture- Density Relationship Test Results) i. The Test Number Bulk-TP2-L1 in the summary table isn’t consistent with that in the lab report. Please clarify and make a correction. ii. Please provide the test result on the fourth layer according to the Table 4.2 of the CQA Manual. 10. (Hydraulic Conductivity Test Results) Please provide the test result on the fourth layer according to the Table 4.2 of the CQA Manual. Appendix H 3 11. (Control Test Summary Table – Grain Size Analysis) i. The lab reports contain the moisture content result for each soil sample, please add the moisture content results to the summary table. ii. The following soil sample identification/test number is inconsistent with that in the lab report:ST-H8-L1 & ST-F3-L4. 12. (Control Test Summary Table – Atterberg Limits) The following soil sample identification/test number is inconsistent with that in the lab report: ST-H8-L1 & ST- E12-L4. Appendix H 4 13. The following sample test results are summarized in the Table but the laboratory data is not included: ST-C9-L4, ST-C10-L4, ST-E10-L4 & ST-G14-L4. Appendix J 14. Please provide a certification or a written document that the project engineer or his/her the designee had visually inspected the constructed compacted clay liner which surfaces are properly prepared and ready to accept the HDPE liner according to the approved CQA plan and Rule 15A NCAC 13B .1624(b)(9)(C)(i). Appendix J4 15. (Geomembrane Seam Destruction Test Log) Please address the following concerns: i. The HDPE samples of DS#27 fail the minimum shear strength of 120 ppi. But the sample is noted as” pass.” Please explain the reason why this sample is acceptable. ii. There are not field destruction test results for shear strength for the following HDPE samples –DS#39 through DS#52 and DS# 58 through DS#72. Please provide data. iii. The shear strength of one of the five coupons of the HDPE sample DS#115 is 3160 ppi. It is likely a typo. Please clarify. Appendix L3 16. Please address the following concerns: i. Eight (8) of ten (10) grab strength results of the geotextile sample (GT-S Type) are less than the minimum criteria of 160 lbs (MARV) specified in Technical Specification Section 02240. Please confirm if the material was rejected from the project. ii. The CQA Plan (Table 7-1) requires the test frequency is 100,000 square feet (SF) or 1 per lot for received geotextile. According to data in Appendices L2 & L3, three CQA tests on the received geotextile (600,000 SF of GT-S and 28,800 SF of GT-C) were conducted.
