The Division of Waste Management (DWM), Solid Waste Section (SWS) has completed a review of the
certified Construction Quality Assurance Report (CQA Report) for the constructed Anderson Creek
C&DLF – Phase III, Cell A, Subcell 2 (Phase IIIA-2). The referenced CQA Report is titled as:

 Construction Quality Assurance Report, Harnett County Anderson Creek C&D Landfill – Phase
IIIA-2. Dated November 2016. Prepared by Smith Gardner, Inc. (DIN 27009).

Based on the reviews, the SWS determines that Harnett County Anderson Creek C&DLF has met the pre-
operative conditions stated in the Permit to Construct dated June 05, 2015 (DIN 24432) and approved
CQA Plan and Technical Specifications (DIN 24434) and hereby, issues Harnett County the Permit
Approval to Operate – Phase IIIA-2 of the Anderson Creek C&DLF (DIN 27031). Please place the
approved documents including the enclosed permit in the facility operating record. Ms. Elizabeth
