STATUS OF PERMIT:
Active, the original PTO was issued on April 4, 1996 and was recorded in Book 581 page 251 of the Pasquotank Co.
Register of Deeds. The current PTO was issued on September 27, 2011 and is for the continuing operation of Phases
1 through 5 including the newly constructed 1.65acre area inside cell 5. The PTO expired on September 27, 2016. An
application for a permit amendment was received on August 16, 2016 and was considered complete per letter dated
September 27, 2016. A technical review is in process.

Partial Facility Compliance Inspection.

ADDITIONAL COMMENTS;
1 – The Permit, Operations Plan and Contingency Plan were posted on site and reviewed. White goods and scrap tire
management are included in the C&DLF permit. Map of Groundwater monitoring and Methane gas wells on site. This
site has made it easy to identify the two types of wells. Groundwater wells are painted Yellow and the Methane wells
are painted Green. All have locks and had easy access. Records available for review.
2 - The operating hours are from 8:00AM to 4:30PM, M-F and 8:00AM to 12PM on Saturday.
3 - The service area includes Pasquotank and Camden Counties.
4 - The permit allows for an average of 65 TPD based on 310 working days.
5 - The facility is being jointly managed by R&D Excavation Co. and Stephenson Sand.
6 – Operator certifications include: Justin Seth Stevenson, expires Feb.26, 2019: Carl Douglas Swain, Jr. expires Dec. 06,
2018: Melvin Owens, expires Aug. 27, 2018: Janice V. Browne, expires April 26, 2019.
7 - Waste screening records are acceptable. Approximately two screenings are conducted each month.
8 – Yard waste area looks good.
9 - A Treatment and Processing area located north of the active C&DLF area is incorporated in the permit. The
operation consists of the temporary storage and composting/grinding of yard waste. The maximum amount of ground
material that can be stored is 4000 cu yards. Piles should be no higher than 12 feet.
10 - A .43 acre wooden pallet storage and processing area is located east of the active C&DLF and can contain up to
2500 cu/yd. Pallets will be ground into mulch for reuse.
11 - An overhead camera was installed to screen incoming loads at the haul road adjacent to the new office. Currently
broken.
12 - The working face is currently located in the newly constructed 1.65 acre area inside cell #5 and is well within the ½
acre requirement. Waste being contained and covered well. Very small amount of windblown waste anywhere on site
noted.
13 - The scrap tire disposal area is incorporated in the permit. Tires are dumped on the ground and then transferred by track
hoe to open top trailers for removal. Tires were on the ground and being loaded during this inspection.
14 – Coverage of LCID area appears to be within 1.5 acres.
15 – Site is grinding concrete for rock and using on roads.
16 – Digital pictures taken.

administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
