STATUS OF PERMIT: C&D Landfill Permit To Operate (PTO) issued August 4, 2014 C&D Permit To Construct (PTC) issued in conjunction with PTO C&D Landfill PTO expiration date January 16, 2024 C&D Landfill PTO subject to limited review January 16, 2019 T&P Operation PTO issued in conjunction with C&D Landfill PTO T&P Operation PTO expiration date January 16, 2019 PURPOSE OF SITE VISIT: Comprehensive Inspection and Pre-operations Meeting STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 6 ADDITIONAL COMMENTS On November 9, 2016, John Patrone and Allen Gaither met with Seth Heath and Catherine Hernandez to conduct a comprehensive inspection and pre-operations meeting of the WI High Point Landfill, LLC - C&D Landfill and C&D Treatment and Processing Operation located on Riverdale Drive in Jamestown, Guilford County. 1. The facility is a construction and demolition debris (C&D) landfill with C&D treatment and processing (T&P) operation. 2. The facility permit approves the operation of the C&D landfill Phase 1, Phase 2A, and Phase 2B-1 and the C&D T&P operation. 3. The permit to construct (PTC) pertains to Phase 2B-2. 4. Phase 2B-2 has been constructed. 5. The SWS issued the facility a letter stating that the pre-operative conditions pertaining to Phase 2B-2 have been met and that the permit will be updated accordingly when the PTC application review for Phase 3 is complete. 6. The facility is accepting C&D in Phase 2B-1. 7. The facility is in operation Monday through Friday 7:00 am – 4:30 pm. 8. The facility permit, site plan, and operations plan were discussed. 9. Phase 1 and Phase 2A are covered with soil and contain vegetative ground cover. 10. The facility is permitted to receive solid waste from Alamance, Cabarrus, Caswell, Davidson, Davie, Forsyth, Guilford, Orange, Randolph, Rockingham, Rowan, Stokes, Surry, and Yadkin Counties. 11. The facility is permitted to receive C&D, inert debris, land clearing debris (LCD), and (used) asphalt. 12. The following are facility certified personnel: a. Catherine Hernandez, Certified Landfill Manager, No. 966207, exp. 02/26/18 b. Joseph Richardson, Certified Landfill Manager, No. 961582, exp. 11/19/17 c. Clyde Herrin, Certified Landfill Operations Specialist, No. LF-2004044, exp. 10/08/19 d. Bengy Austin, Certified Landfill Operations Specialist, No. LF-2013065, exp. 10/18/19 13. The facility is required to conduct one waste screening per week but > 1% by weight. Waste screening records were verified for January through November 8, 2016. 14. The facility maintains a record of the amount of material received. Throughput records for January through November 8, 2016 were verified. The facility received 14,637.89 tons of C&D at the landfill, 366.66 tons of C&D at the C&D T&P operation, 231.29 tons of C&D fines, 36.44 tons of asbestos, 30.65 tons of LCD, and 663.26 tons of concrete. 15. The facility annual report (FAR) for the C&D landfill was received by the SWS, dated July 25, 2016. Tonnage received July 2015 through June 2016 is 36,715.67. 16. The FAR for the C&D T&P operation was received by the SWS, dated July 25, 2016. Tonnage of C&D at the C&D T&P operation received July 2015 through June 2016 is 4,685.21 (57.22 tons of LCD, concrete, and metal were also received). 17. A vehicle scale test was conducted by J.A. King and Company LLC on July 14, 2016. 18. A vehicle scale test was conducted by the North Carolina Department of Agriculture & Consumer Services in August 2015. 19. The facility scale has an outside weight display unit. 20. The working face was small and compact. 21. The landfill footprint is designated by edge of waste markers. 22. Edge of waste markers have been installed at the newly constructed Phase 2B-2. The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 3 of 6 23. The facility does not actively use alternative daily cover (ADC). Mr. Heath stated that a soil/mulch mix is used for several weeks of the year, generally in the spring and fall, and the C&D fines are used intermittently. If the facility plans to use soil/mulch mix and C&D fines as ADC it must be incorporated into the facility operations plan and listed in the permit at the next permit action. Contact Allen Gaither, SWS - Environmental Engineer at 828-296-4703, for further discussion. 24. The C&D T&P picking line was not in operation during the inspection. 25. Mr. Heath stated that the C&D T&P picking line is operated as needed. 26. The C&D T&P operation consists of a conveyor-fed raised picking line and materials reclamation pad. 27. Mr. Heath stated that land clearing debris is ground in the spring and fall and concrete is crushed twice a year. 28. In order to operate the C&D T&P picking line, the facility places suitable C&D in roll-off containers. The roll- off containers are covered with a tarp. At least eight roll-off containers of material is needed to operate the C&D T&P picking line for a day. The facility shall not store C&D directly on the reclamation pad if the picking line is not in operation. Ensure the C&D T&P pickling line operation is updated in the facility operations plan at the next permit action. 29. The facility has an asbestos operations plan on file for the C&D T&P operation. 30. The facility maintains records of reclaimed materials brought to end-users, processors, and recyclers. Crushed concrete is used on site as a road base material, scrap metal is hauled to D.H. Griffin Companies, cardboard and plastic is brought to the Waste Industries USA, Inc. materials recovery facility in Greensboro, and ground wood and C&D fines are used at the landfill as ADC. Mr. Heath stated that no cardboard and plastic has been reclaimed in 2016 and that 13.46 tons of scrap metal has been reclaimed. 31. Post consumer recycled asphalt shingles (PRAS) are not currently accepted for reclamation. 32. The facility has an asbestos operations plan on file for the PRAS operation. 33. Mr. Heath stated that when material known to contain asbestos is received it is placed in a hole, covered with soil, and the GPS location obtained. 34. The facility maintains a record of the asbestos containing material received at the landfill. 35. The facility shall cover solid waste with six-inches of soil when the waste disposal area exceeds one-half acre and at least once weekly. 36. The facility maintains a weekly soil cover log. 37. The facility maintains semiannual groundwater and surface water monitoring test records submitted by Golder Associates NC, Inc., analyzed by Environmental Conservation Laboratories, Inc. (ENCO). Records for February 17-18, 2016 and July 14-15, 2016 were verified. An exceedance is noted for each monitoring event. 38. The facility conducts groundwater monitoring at eight monitoring well locations: MW-1, MW-2, MW-3, MW-4S, MW-5, MW-6, MW-7S, and MW-8. Surface water monitoring is conducted at four surface water locations: SW-1 through SW-4. 39. The SWS approved the facility assessment monitoring notification and work plan addendum in response to groundwater volatile organic compounds (VOCs) exceedances. 40. The facility will install a permanent landfill gas monitoring well (LFG-1), two groundwater monitoring wells (AMW-1 and AMW-2), and conduct Appendix II monitoring. 41. The facility will begin assessment monitoring for VOCs during the first quarter of 2017. 42. The facility maintains quarterly methane monitoring records. Methane monitoring is conducted by Golder Associates NC, Inc. Records were verified for February 3, 2016 (1st quarter), June 1, 2016 (2nd quarter), July 18, 2016 (3rd quarter), and October 14, 2016 (4th quarter). No exceedance of the methane lower explosive limit (LEL) is noted. 43. The facility also monitors for hydrogen sulfide. No exceedance of the hydrogen sulfide LEL is noted. 44. The facility conducts landfill gas monitoring using a bar hole punch. Bar hole punch tests are conducted at five locations: BH-1 through BH-5. It is suggested that the bar hole punch test locations be permanently marked. 45. Maintain a readily accessible unobstructed path to each groundwater monitoring well, surface water location, and landfill gas bar hole punch test location. 46. The facility has a stormwater National Pollutant Discharge Elimination System (NPDES) permit, General Permit No. NCG120000, Certificate Of Coverage No. NCG120099. The NPDES permit expires October 31, 2017. 47. The landfill cap appeared in good order. It contained vegetative ground cover. 48. The facility has a soil stockpile from operations conducted on site. Mr. Heath stated that the landfill is mowed twice a year. 52. Mr. Heath stated that the facility uses a water truck for dust control. 53. The facility does not accept scrap tires or white goods. 54. Facility financial assurance cost estimates have been approved. 55. The facility has all-weather access roads. 56. The facility is secured by a locked gate. 57. The facility has a proper sign. 58. The C&D landfill PTO expiration date is January 16, 2024. 59. The C&D landfill PTO renewal application shall be submitted to the SWS by July 16, 2023. 60. The C&D landfill PTO is subject to limited review by January 16, 2019. 61. The C&D landfill PTO limited review request is due by July 16, 2018. 62. The T&P Operation PTO expiration date is January 16, 2019. 63. The T&P Operation PTO renewal application shall be submitted to the SWS by July 16, 2018.
