(S+G) would like to respond to the comments in your email dated and received by S+G on November 7, 2016 (see attached). Your comments are repeated below in italics followed by our response in bold. 1. The approved CDLF - Phase IIIA acreage is 2.1 acres according to the Permit to Construct Application (DIN 24434) which is incorporated into the Permit To Construct for Phase 3A (DIN 24432). However, according to the certified CQA Reports for Phases IIIA the total constructed acreage is 2.34 acres consisting of 0.54 acres of Phase IIIA-Subcell 1 (DIN 25058) and 1.8 acres of Phase IIIA- Subcell 2 (DIN 26986). Please note the variance to the CQA Report for Phase IIIA-2. The calculated 2.34 acres (or 2.3 acres if rounded to nearest tenth) for Phase IIIA has been double checked and is noted in Section 2.1 of the enclosed revised CQA Report. S+G also checked the areas of Phases IIIB, IIIC, and IIID (see attached calculations). Phase IIIB is actually 1.84 (1.8 acres rounded) instead of 1.9 acres shown in the permit. The areas of Phases IIIC and IIID are correct (1.9 and 1.6 acres, respectively). The minor differences in area for Phases IIIA and IIIB are believed to be rounding errors at the time of original calculation. In concert with the response to Comment No. 5 below (Phase IIIA capacity), attached is an updated capacity table showing the revised areas and Phase IIIA capacity. 2. (Appendix C) According to the certified CQA Reports, two Standard Proctors – moisture & density relationship tests by ASTM D698 were conducted for the project, but there are another twelve (12) referenced Standard Proctor tests listed and referenced in the Structural Fill Record Test Summary in Appendix C of the CQA Report. Please provide these twelve (12) referenced Standard Proctor test results. GeoTechnologies’ technicians performed one-point Proctor tests in the field when performing in-place density testing and the one-point result was plotted in relationship to the closest laboratory Proctor curve for the project. November 10, 2016 Page 2 of 3 then used the maximum dry density from the plotted one point, instead of the actual Proctor curve maximum dry density as, per GeoTechnologies’ opinion, the result is felt to provide a more accurate result at the end. Thus, the reference standards are mainly from the one-point tests. 3. (Appendix C) The compaction effort of the test sample 26 is approximately 94.6%, which is less than the specified 95%. The Structural Fill Record Test Summary in Appendix C list the test result as “Fail” but re-test didn’t performed. If the Engineer considers the test result is a marginal value but acceptable (Clause of Engineer Judgement), please state in the CQA Report. Otherwise, please provide the retest and pass result. The result (94.6%) for Test Number 26 was intended to be deemed acceptable as the result rounds to the required 95%. The Structural Fill Record Test Summary in Appendix C has been revised accordingly in the enclosed revised CQA Report. 4. (Appendix C) In-Place Density Test Results dated 09/13/2016, the test sample 32a is likely a typo. Please clarify. Yes, “32a” is a typo in the 9/13 report (should have been “33a”). The Structural Fill Record Test Summary labels this test result as “33a”. The 9/13 report has been marked accordingly in the enclosed revised CQA Report. 5. (Figure AB-1) The constructed base grades for the Phase IIIA-2 are different from those shown on Sheet 5/Drawing No. S4 in the approved PTC Application for Phase III (DIN 24434). The lowest base grade for the constructed Phase IIIA-2 is 310 feet as shown on the as-built drawing Figure AB-1 rather than 300 feet. If the survey data is correct, the operating capacity for the new subcell should be reduced significantly; and new capacity data should be provided to the report which will be incorporated into the Permit to Operate for this new subcell. Please confirm the base grade elevations on the as-built drawing. The lowest elevation of Phase IIIA-2 is actually 302 at the southwest end (2 foot contours are lighter than 10 foot contours). As noted in Section 3.2 of the CQA Report, this area was purposely constructed higher than permitted grades to stay sufficiently above wet areas encountered during placement of structural fill. Based on the as-built base grades for Phases IIIA-1 and IIIA-2, the revised gross capacity of Phase IIIA is 94,089 CY (slightly less than the permitted 96,412 CY) (see attached calculations). Harnett County AC C&DLF - Phase IIIA-2 Construction Quality Assurance Report November 2016 (Revised) Page 1 1.0 OVERVIEW This Construction Quality Assurance (CQA) Report has been prepared to document the CQA activities performed during the construction of Phase IIIA-2 of the Anderson Creek Construction and Demolition Debris (C&D) Landfill. The landfill facility is located at 1086 Poplar Drive in Spring Lake, North Carolina and is owned and operated by Harnett County under State Solid Waste Permit No. 43-03. A Permit to Construct for the Phase IIIA landfill unit was issued by the North Carolina Department of Environmental Quality Division of Waste Management (NCDWM) on June 5, 2015. 2.0 PROJECT DESCRIPTION 2.1 General Phase IIIA-2 is an unlined C&D landfill unit designed by Smith Gardner, Inc. (S+G). This unit, which is part of the larger Phase IIIA landfill unit, is approximately 1.8 acres* (limits of certification) and is located to the northeast of the existing Phases I, II, and IIIA-1 units. Construction activities included of subgrade preparation activities, and construction of a small berm. *Note that the combined area of Phases IIIA-1 and IIIA-2 is actually 2.34 acres, which is slightly larger than 2.1 acres shown in the approved Permit to Construct application. 2.2 Reference Documents Phase IIIA-2 was constructed in accordance with the following documents. Photos documenting the construction of Phase IIIA-2 can be found in Appendix A. Prior to beginning the project, a CQA meeting was held on July 7, 2015 (and repeated on August 23, 2016). Documentation of this meeting can be found in Appendix B. 3.1 Site Preparation Construction of Phase IIIA-2 began in August 2016 with the surveying/staking of the limits of construction by 4D Site Solutions and the initiation of earthwork activities by County forces. 3.2 Earthwork The site, which lies within a portion of the site designated for borrow, was previously cleared and grubbed. Additionally, an existing French drain running northeast of and adjacent to the Phases I & II Extension was previously removed to the extent practical and the remainder abandoned (References: NC DWM DIN 21375 (Revised Report with Documentation) and DIN 21384 (Acknowledgement of French Drain Removal)). Once the site was staked, excavation and stockpiling activities were performed. Suitable soils identified for use as structural fill were excavated and placed and compacted or were temporarily stockpiled. A portion of the structural fill came from the County’s nearby active borrow area. During construction of the subgrade for Phase IIIA-2, Mr. Pieter Scheer, P.E. visited the site and examined the subgrade in accordance with the requirements of 15A NCAC 13B.0540. As anticipated based on the site investigations performed in this area, no evidence of bedrock or groundwater was observed in excavations made to reach subgrade elevations. An as-built drawing showing completed subgrade elevations is provided in Appendix D. Permitted seasonal high groundwater contours are also shown on this drawing to illustrate the separation to groundwater. It is noted that much of the southwestern portion of Phase IIIA-2 was constructed higher than design grades to stay sufficiently above wet areas encountered during placement of structural fill. Harnett County AC C&DLF - Phase IIIA-2 Construction Quality Assurance Report November 2016 (Revised) Page 4 4.0 CQA PROGRAM 4.1 Scope of Services In satisfying the requirements of the Project CQA Manual for Phase IIIA-2, the following activities were performed:  Observation and documentation of construction of prepared subgrade and structural fill.  Field and laboratory testing of structural fill.  Verification of the soil types in the upper two (2) feet of the subgrade.  Review/preparation of record drawing.  Preparation of the final CQA report. 5.0 EARTHWORK CQA The criteria for construction of structural fill per the project specifications included the following: Materials: SP*, SW*, SM, SM-SC, SC, ML, MH, ML-CL, CL, or CH (ASTM D 2488) with no topsoil or other deleterious material and no stones or rocks in excess of one half the lift thickness as compacted; Density: Minimum 95% Maximum Standard Proctor Dry Density (ASTM D 698); Moisture Content: As necessary for compaction; and Lift Thickness: 8-inch max. (compacted). *Within the limits of the landfill, SP (poorly-graded sand) and SW (well-graded sand) soils are not allowed in the upper two (2) feet of the subgrade (see Section 5.1). The number and results of material control and record tests performed on the structural fill are summarized in Table 1. Other tests performed on an on-going basis during construction included a visual classification of soils (ASTM D 2488) and monitoring of loose lift thickness. Note that the number of tests required was based on an approximate quantity of 10,000 CY of material placed (in-place measure). The results of field and laboratory testing of structural fill can be found in Appendix C. An as-built drawing showing completed subgrade elevations is provided in Appendix D. Harnett County AC C&DLF - Phase IIIA-2 Construction Quality Assurance Report November 2016 (Revised) Page 5 5.1 Soil Types in Upper Two Feet of Landfill Subgrade For this site, no SP, SW, or coarser soil types (as defined using ASTM D 2488) were found to be present within the limits of Phase IIIA-2 based on visual inspection and testing performed as part of construction. Additionally, the County generally excavated to two feet below design subgrade in cut areas such that materials could be monitored during placement. Five tests performed on actual materials placed in the upper two feet of the landfill subgrade show that the soils used were classified as SM of SC. These soil types meet the requirements of 15A NCAC 13B.0540 for the upper two feet of the landfill subgrade. Given that the materials tested appeared to be representative of all materials placed as structural fill during construction, the five material classification tests were deemed to be sufficient. 6.0 RECORD DRAWING The following record (as-built) drawing depicting the construction of Phase IIIA-2 can be found in Appendix D:  Subgrade (prepared by S+G using surveyed elevations by 4D Site Solutions, Inc., Jimmy Holland, PLS). The meeting began at approximately 2:00 p.m. and lasted approximately 30 minutes. The County will contact Streamline Land Surveying (Robert Godwin, PLS) for staking of the site. Streamline will also prepare an as-built survey at the completion of the work. The County will contact S+G/GeoTechnologies to perform moisture/density testing of structural fill placed. Earthwork testing and subgrade verification will follow the attached specification and CQA requirements (anticipated earthwork quantities are identified also). S+G will prepare the certification report which will include the as-built survey and information from tests performed by GeoTechnologies.
