He has extensive experience in the on site supervision of safety, quality control, and all required documentation. In addition to his supervisory experience i also has actual hands on installation experience which far exceed all of the requirements to be qualified as a Leadman, Quality Control Technician and Master Seamer. As the on site ESI Superintendent he is also responsible for the direct, day to day, on site safety management of the crew. Foothills Regional MSW Landfill – Caldwell Co., North Carolina May 24, 2016 Report of Geologic Observation: Cell No. 5A BLE Project Number J15-1235-75/-76 1 1.0 PROJECT INFORMATION The existing Foothills Regional Municipal Solid Waste (MSW) Landfill is located on Cheraw Road approximately two miles northwest of Lenoir, North Carolina (Figure 1). The landfill is owned by the Caldwell County Board of Commissioners and operated by Republic Services of North Carolina, LLC (Republic). The landfill development is being implemented in phases as new solid waste cells are needed. To date, Cell Nos. 1A, 1B, 2A, 2B, 3A, 3B, 3C, 4, and 8A have been constructed. Cell No. 5A is currently under construction and is the subject of this report. BLE personnel are familiar with the site and previously prepared the Design Hydrogeologic Reports (DHR)1 2 3 4 and provided CQA services for the construction of Cell No. 2, Phase 2 (Cell No. 3), and Phase 3 (Cell Nos. 4 and 8A). This report documents our observations of the Cell No. 5A subgrade during cell construction and also documents rock blasting activities in proposed Cell No. 5A, future Cell Nos. 5B and 6, and Sediment Basin C. 2.0 SITE GEOLOGY The crystalline rocks of the Inner Piedmont Belt occur in generally northeast-southwest trending geologic belts in the Carolinas, and consist of a stack of highly metamorphosed thrust sheets bound on the northwest by the Brevard Shear Zone and to the southeast by the Kings Mountain Shear Zone. The Inner Piedmont Belt includes high-grade metamorphosed sedimentary and igneous rocks that have been exposed to multiple deformations. Rock types that resulted from the multiple metamorphisms include gneiss, schist and amphibolite with northeast/southwest trending foliation with varying degrees of dip. Quaternary-age sediments consisting of sand and gravel fill the stream valleys. The subgrade geologic conditions across the site are often variable with boulders and seams of partially weathered rock occurring throughout the subsurface soil overburden profile. In general, three zones were encountered by the borings performed for the DHR in the Cell No. 5A area: 1) the residual soils from weathered gneiss and schist, 2) the partially weathered rock, and 3) the fractured gneiss and schist bedrock. Also, narrow and thin zones of alluvial sediments may occur in the topographic drainage features across the site. Fractures, joints, and the presence of less resistant rock types facilitate weathering. Consequently, the profile of the partially weathered rock and hard rock is quite irregular and erratic, even over short horizontal distances. Also, it is not unusual to find lenses and boulders of hard rock and zones of partially weathered rock within the soil mantle, well above the general bedrock level. Foothills Regional MSW Landfill – Caldwell Co., North Carolina May 24, 2016 Report of Geologic Observation: Cell No. 5A BLE Project Number J15-1235-75/-76 2 The residual soils are the result of the in-place weathering of the gneiss and schist bedrock. The residual soil profile below the topsoil consists of two identifiable components based on the Unified Soil Classification System (USCS). An upper soil component consists of reddish-brown, micaceous silty clay and sandy clayey silt with USCS classifications of ML, MH, and CL. The upper soil component grades with depth into a coarser grained, less plastic, brown, gray, and white micaceous sandy silt and silty sand with USCS classifications of ML and SM, which extends to the depth of the partially weathered rock and/or bedrock. Some gravel and boulder size rock fragments were common in both soil components. 3.0 FIELD OBSERVATIONS Mark S. Preddy, P.G., Tyler W. Moody, P.E., and Daniel B. Bunnell, P.E. of BLE visited the site on multiple occasions during the construction of Cell No. 5A and during the rock blasting activities from June 2015 through April 2016. During these visits, the exposed ground surface was observed to identify anomalous geologic features not identified during previous geologic investigations, as well as to document the rock blasting activities. 3.1 Cell No. 5A Observations The July 2015 through January 2016 site visits were conducted during Cell No. 5A grading activities and limited rock blasting. The February 2016 site visits were conducted near the completion of the structural fill subgrade preparation prior to structural fill placement. Site observations are summarized below and are shown on the attached photographs 1 through 12 in Appendix A. In addition, the BLE CQA engineering technician was on site during the cell rough grading and cell subgrade construction. The completed cell excavation was proof-rolled with a loaded dump truck and then compacted using a footed compactor and a vibratory drum roller. The Cell No. 5A footprint covers approximately 6.4 acres. The northern end of the cell is in an upland location that slopes westward. The southern end of the cell was formerly the location of Sediment Basin No. 2, which was removed during the construction of Cell No. 5A. Much of the topsoil and shallow clayey soils were already removed from the Cell No. 5A area prior to current cell construction activities. The cell subgrade was formed: 1) by excavation into the residual soils; 2) by localized and limited rock blasting of rock material too hard to be removed by three passes with a D-8 dozer equipped with a single tooth ripper; 3) by removing the loose sediments, land clearing debris, prior fill, and accumulated organics from Sediment Basin No. 2 formerly in the southern end of the Cell No. 5A footprint; and 4) by placing structural fill, where necessary. The soils exposed at the subgrade level were primarily the sandy and micaceous portions of the saprolitic residual soil profile, which is consistent with prior descriptions of the site (Appendix A, Photos 1, 3, and 7). The metamorphic foliation in the saprolite and weathered rock strikes northeast, and dips to the southeast, which is consistent with trends measured at the site and documented in prior DHR reports (Appendix A, Photo 9; Appendix B). Additionally, the weathered rock fractures (joints) strike northwest and dip steeply to the northeast and southwest, INC. Loose sediments, organic matter, and land clearing debris had been previously deposited in the former ravine underlying Sediment Basin No. 2 formerly located in the southern portion of Cell No. 5A (Appendix A, Photos 2 and 8). These materials were removed from the former ravine down to the residual soils. Where necessary to reach design subgrade elevations, these areas where loose sediments and land clearing debris were removed, were backfilled by placing and compacting approximately 10-inch thick lifts of structural fill soils and up to 2-foot thick lifts of rock with soil fill in accordance with the project technical specifications and the CQA Manual (Appendix A, Photos 11 and 12). Weathered rock material resistant to excavation was encountered in the northern end of the Cell 5A footprint (Appendix A, Photos 1, 3, 5, and 6). Rock blasting was performed to reach the desired elevation of greater than 4 feet below the proposed soil liner subgrade. Rock blasting reports are included in Appendix D. The general contractor for the Cell No. 5A construction, Preston Contractors, provided elevation data for each structural fill density test location referencing the depth below the design subgrade elevation (bottom of soil liner). The elevations provided along with the observed thickness of the soil being placed to achieve the subgrade elevation confirmed 4 feet or more of structural fill soil overlying the rock. The density test results will be provided in the Cell No. 5A CQA Report. No other pertinent geologic features were encountered during cell subgrade preparation. The placement of structural fill on top of the residual soil and bedrock began in late February 2016, followed by the placement of the compacted soil liner in early May 2016. 3.2 Rock Blasting Observations Pinnacles of rock were encountered above the general bedrock level documented in the DHR report. As a result, rock blasting was performed in three primary areas: 1) the northern end of Cell No. 5A and southern end of future Cell No. 5B; 2) the southern end of future Cell No. 6; and 3) the area near Sediment Basin C (Figure 2). A blasting plan was prepared by JT Russell and Sons, Inc. and submitted to the North Carolina Department of Environmental Quality (NCDEQ) on June 15, 2015 (Appendix C). Twenty-three (23) rock blasting events were performed between June 24, 2015 and April 13, 2016 by J.T. Russell and Sons of Albemarle, North Carolina (blasts No.1 through 18) and by Preston Contractors of Kingwood, West Virginia (blasts No. 19 through 23). The approved blast plan submitted by JT Russell Sons for blasts No. 1 through 18 was reviewed by Preston Contractors and incorporated for blasts No. 19 through 23 by Preston. Vibration monitoring was performed by Vibra-Tech Engineers, Inc. of Louisville, Kentucky. The rock blasting reports are included in Appendix D and include information regarding: 1) the date, time, location of the blast; 2) the number of blast holes including their spacing and depths; 3) explosive material used, and 4) vibration monitoring results. Vibration results were provided to BLE for review following each blast event by Vibra-Tech. Nine vibration monitoring points were selected and used interchangeably as appropriate, which included the edge of geomembrane at Cell Nos. 3C, 4, and INC. Foothills Regional MSW Landfill – Caldwell Co., North Carolina May 24, 2016 Report of Geologic Observation: Cell No. 5A BLE Project Number J15-1235-75/-76 4 8A (5 total locations), MW-1, GP-12, MH-1 (piezometer PZ-108C), and at the site maintenance building (Figure 2). Each of the reported vibration results were less than the 2 inches per second peak particle velocity shown in the approved blasting plan for this project with the exception of the blast on November 25, 2015. The vibration result recorded during the blast on November 25, 2015 at the edge of geomembrane in existing Cell No. 3C was unusually high (approximately 5 inches per second). A post blast inspection of the vibration monitoring location revealed that the seismograph encasement had been struck and knocked over by apparent fly rock; therefore, this vibration result was determined to be not indicative of the actual ground vibrations. The remaining vibration measurements were less than the established project threshold value of 2 inches per second. The blasting broke the in-place rock into manageable pieces of rock that were crushed on site by JT Russell and Sons, Inc. for possible use for site construction activities (Appendix A, Photos 3, 4, 13, 14, 15, and 16). Material from blasting activities was also used to backfill the over-excavation of former Sediment Basin No. 2. All rocky fill was transitioned to a soil structural fill a minimum of 5 feet below the design Cell No. 5A subgrade (Appendix A, Photo 11). 3.3 Termination of Existing Underdrain Pipe The existing underdrain pipe beneath the subgrade of Cell No. 2B was capped by Preston Contractors as a part of the subgrade preparation for Cell No. 5A. In accordance with the approval letter from NCDEQ (Appendix E), the underdrain pipe was terminated a vertical distance of greater than 4 feet beneath the Cell No. 2B subgrade using a glued PVC end cap which was then encased in concrete. 4.0 CONCLUSIONS Loose sediments, organic matter, and land clearing debris formerly present in the cell footprint as a result of the Sediment Basin No. 2 were removed during cell subgrade preparation. Limited rock blasting was performed to achieve the desired cell subgrade level in isolated areas. The geologic conditions observed in the Cell No. 5A subgrade were generally consistent with prior observations at the site and the DHR. Therefore, we conclude that modification of the existing Water Quality Monitoring Plan is not necessary. FIGURES APPENDIX A Photographs INC. J. T. Russell and Sons, Inc., plans to blast within the limits of construction and the areas identified as rock on the project. Only in the areas where the rock cannot be excavated by conventional means will blasting occur. JTR will work closely with the owner and engineer during the blast phase. 1) TYPES OF BLASTING Primary type of blasting will be for excavation removal purposes. Blasting may also require blasting in storm drain trenches. 2) SAFETY PROCEDURE FOR BLASTS All operations for transporting, handling, and loading explosives shall observe applicable industry standards and regulations as defined by the appropriate city, county, state, or federal governing agency. All blast events will be conducted in such a manner that complies with all safety cautions, procedures or regulations of the applicable city, county, state, or federal governing agency. This includes, but is not limited to: clearing the blast zone, guarding the blast area and vehicular or pedestrian traffic control. There will be no on-site, overnight storage of explosives. 3) BLAST SITE DIMENSIONS The blast will consist from 50-300 holes covering approximately .1-2 acres per rock identified areas. The depth of each borehole will be around 6-30 +/- feet including sub drill. An estimated 35,000 CY of blasting will take place. 4) DRILL PATTERN The proposed pattern is to drill 3” - 4.5” diameter boreholes with a pattern of 5’x5’ or 10’x12’. 5) EXPLOSIVES LOADING AND INITIATION a)It is proposed to use a booster for the main charge in each borehole, initiated with an appropriate detonating blasting cap. This cartridge weighs approximately 1 pound per for 5 inches of 2-inch borehole. Surface (non-electric) delays will be used to minimize the vibrations at adjacent structures within 500 ft. The blast delay intervals used will be JTR BLASTING PLAN FOOTHILLS REGIONAL MSW - LENOIR LANDFILL 9ms, 17ms, 25ms, and 42ms. Each blast will be designed to have a max of 1-500# pounds of explosive per delay. b)As one means to control ground movement, a minimum of 4’ of inert stemming material will be loaded at the top of each borehole, beginning below the depth where competent rock was encountered. Stemming height will be determined by the discretion, experience and judgment of the blaster in charge, and site-specific conditions for each shot. 6)GROUND CONTROL All blasts will be designed to prevent excessive movement of material from the blast zone. The blaster will include in his considerations such factors as rock type, geologic condition, borehole depth, charge weight and any other relevant, site specific conditions for each shot. 7)FLYROCK CONTROL PLAN All shots will be carefully designed by the qualified blaster to control fly rock. All hole loading activity will be supervised by the qualified blaster. The qualified blaster will communicate with the driller to obtain geological information for each shot. JTR will haul and place soil over blast area for a protective cover from fly rock. 8)SELECTION OF BLASTING PRODUCTS AND METHODS These blasting products were chosen because of many years of dependable use and positive results which are demonstrated by the: •Quality, safety and reliability of the product •Support offered by the manufacturer •Availability •Price 9)MONITORING BLAST EFFECTS It is not expected that this type of rock will fracture in such a way as to cause any kind of ground displacement. Following each blast, the area will be examined for signs of ground cracking. Any indication of over break will be brought to the attention of the blaster and noted on the blast report. Vibration monitoring will be conducted at the structures located nearest the blasting. The vibration levels produced by the blasting are estimated to be less than 2 inch per second PPV and 132 dBI at the closest location. The blasting will be monitored at the buildings closest to the blast up to 500 ft or as advised. The blaster in charge will design each blast to minimize vibration and over pressure and to prevent fly rock. 10) PEAK PARTICLE VELOCITY MONITORING AND CONTROL Each blast will be monitored by a licensed blaster and Vibra-tech in monitoring blasts using 3 seismograph machines. The seismograph will be placed at the “point of interest”. In most cases, this will be next to the foundation of the closest building, power line foundation, along edge of line, utility or well. In all cases, both the sensor and seismograph will be protected from fly rock. 11) BLAST WARNING SIGNALS Warning signals for each blast will be as follows: •One long horn one minute before blast •One long horn 10 seconds before blast •One long horn for all clear after the blast 12)ENVIRONMENTAL CONCERNS The Caldwell County Fire Marshal will be notified prior to blasting. 13)FIRE PREVENTION Following the required waiting period after each shot, the blast area will be inspected for any indication of fire or fire hazard. This is a solid section of pipe that was extended for a future tie-in (if required). Since we do not need to extend, or tie into, this existing underdrain piping as part of this project, Republic plans to have this pipe capped on the upper end. Please note that the existing underdrain pipe was capped just outside the limits of Cell No. 2B along the tie-in of Cell No. 5. The purpose of these changes is to lower the pipe so that minimum separation is maintained under the new cell. This will be accomplished by excavating down and cutting the pipe to maintain a min. 4’ of separation between the underdrain pipe and design subgrade elevation. We’re proposing to glue a slip cap onto the exposed pipe end. Concrete may be used to encase the entire cap and pipe end, if it is deemed necessary in the field to completely seal off this pipe. This will eliminate the need to use a welding machine to fuse a cap onto the pipe and a potentially un- safe working condition. As requested, I’ve attached a sketch for your reference. This plan sheet was obtained from the approved Phase 4 Environmental Monitoring Plan originally prepared by BLE—I have added some notes/comments for clarification on this particular item. As you can see, the existing underdrain pipe will remain functional and continue serving Cell 2B.
