STATUS OF PERMIT: Original Permit issued December 12, 2000 Current Permit issued April 6, 2015 Permit Expiration Date April 6, 2020 PURPOSE OF AUDIT: Partial Inspection STATUS OF PAST NOTED VIOLATIONS: NA OBSERVED VIOLATIONS None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE AUDIT REPORT Division of Waste Management Solid Waste Section Page 2 of 4 AREAS OF CONCERN AND COMMENTS: 1. The purpose of this visit is to follow up on corrective actions needed as a result of the last inspection. 2. A ground cover sufficient to restrain erosion needs to be provided on all previously disturbed areas. As a minimum, the provisions to include seed and mulch need to be provided on all bare areas that have not been re-graded for 30 days. I would strongly recommend taking advantage of the spring and summer planting season. 3. Tire Certifications are being properly filled out with original signatures. 4. Daily cover logs noting a date and time of placement was reviewed and is being properly recorded. 5. A detail has been provided that shows the location and design of a sub-surface drain installed to collect a leachate seep and drain into the leachate collection system. 6. The solidification pit was reviewed. The area was graded and leveled out for better management. 7. The Emergency Action Plan or Contingency Plan was updated with a current call list in the event of an emergency. The Plan has been posted in the break room for constant viewing and easy access. 8. Scott Brown response to the Landfill Gas Monitoring. “Due to the natural barriers for landfill gas migration on the northwest and east, Waste Connections is proposing to install one landfill gas monitoring well generally in the area of the leachate tanks.” 9. One leachate tank is not in use. Information is to be provided to John Murray that will resolve the tank not being in use. 10. Gas monitors are to be provided in the building beside the gas flare and the building near the leachate tanks. 11. The Yard Waste area has been graded and cleaned up free of unacceptable waste streams. A traffic pattern and a wood yard log was created to assist with better management of this area. 12. The sediment and erosion control measures have been cleaned free of litter and some areas regraded. 13. The windblown litter has been picked up and is being managed better. I would recommend having a contingency plan in place to handle extreme conditions such as a temporary service to assist. Windblown litter is to be picked up at the end of the day and returned to the working face. I would recommend that litter fencing be used to assist with this effort. 14. A new sign is being made to show updated information. 15. The monitoring wells have all been cleaned off free of silt and vegetation. The wells have all been provided with new locks. 16. Much progress has been made to correct issues noted during the past inspection. Good Job! 17. Continued actions are needed to establish an adequate ground cover sufficient to restrain erosion. Bare or eroding areas outside the working face and adjacent areas need continued efforts to establish a vegetative ground cover. 18. Digital photos were taken during the inspection.
