Division Manager STATUS OF PERMIT: Active, permit issued on August 20, 2013 and shall expire on August 20, 2018, on or about February 20, 2018 the owner or operator must submit a request to the Section for permit review. PURPOSE OF SITE VISIT: Comprehensive Compliance Inspection. STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS: 1 - This facility has a gated entrance. 2 - Signage is posted on the entrance gate with information required except the permit number. 3 - Mr. Holmes to add permit number to signage. 4 - Hours of operation are 7:00 am – 4:30 pm Monday – Friday and 7:00 am – 12:00 pm Saturday (Seasonal). Closed Christmas Day, New Year’s Day and Thanksgiving Day. 5 - This facility has 4 certified operators. Kimberly D. Dalgo – expires 10-9-2016, William W. Harrelson – expires 4-8- 2018, David R. Parker – expires 4-8-2018 and Edward G. Holmes – expires 4-8-2018. 6 - The service area is Camden, Currituck, Dare, Pasquotank, and Perquimans Counties. 7 - The waste stream consists of construction and demolition waste yard debris, single stream recyclables and dry recyclable waste. Recyclables other than C&D will be deposited on the floor in a separate area and loaded into open top trailers for transport to the Hampton Material Recovery Facility. C&D will be deposited on the tipping floor in a designated area loaded into open top trailers and hauled to the Hampton Roads Recovery Center. Incoming loads will be inspected by the driver, facility operator and equipment operators to minimize chance of unapproved materials being accepted 8 - Waste is being transported to the Centerville Turnpike CDD Landfill, Virginia Beach Va. (Virginia Permit Number SWP 603.) 9 - Roads are made of all weather material. 10 - Records of waste received (weight & type) and waste transfer out were kept in good order. 11 - Weigh scales were calibrated by NCDA&CS on 12-15-2015. 12 - Facility is kept very clean. 13 - Scrap metal is currently stored in metal containers. 14 - Non Recyclable items also put in containers for transfer to proper landfill. 15 – Wind blown trash is being take care of. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
