Dear Mr. Czop:
The purpose of this letter is to provide Duke Energy Carolinas, LLC with the documents to meet
requirements #11 and #12 of the Phase 2 Permit to Construct application. Most of the requirements
have already been provided to the Solid Waste Section (SWS) in previous submittals, but for the purpose
of this letter, the required documents have been included as one comprehensive submittal.
Item #11 requests that within 30 days of monitoring well installation, the following information be
submitted to the SWS: A well construction record (GW-1 form), a typical well schematic, boring logs,
field logs and notes, and a description of well development activities. Copies of the well construction
records, well schematics, and boring logs for the newly installed monitoring wells CCPMW-4, -5, and -
6s/-6d are provided in Attachment 1. Copies of the field logs and notes are provided in Attachment 2. A
description of the well development activities follows:
Following installation of each new wells, the wells were developed by air lifting and overpumping. Well
yield for wells CCPMW-4, -5, and -6d was poor and each well was pumped dry a minimum of three
times. Well yield for CCPMW-6s was excellent and the well was pumped until the water was visibly clear
and free of sediment, in accordance with the approved Water Quality Monitoring Plan for the site.
Item #12 requests a plan sheet-sized, scaled topographic map, showing the location and identification of
new, existing, and abandoned wells and piezometers after installation of the groundwater monitoring
wells. A comprehensive well map for the site (Sheet 1) is provided in Attachment 3.
