A complete permit review request including updated facility, operations, and waste screening plans was due on or before 23 June 2013; this request was submitted on 1 May 2013 and received by the Solid Waste Section on 2 May 2013. Authorization to continue operations under the expired permit issued 17 December 2013. PURPOSE OF SITE VISIT: Partial Facility Compliance Inspection without records review STATUS OF PAST NOTED VIOLATIONS: None applicable OBSERVED VIOLATIONS: No violations were observed during this Facility Compliance Inspection; please see the ADDITIONAL COMMENTS section below. ADDITIONAL COMMENTS: 1) This Facility is a Municipal Solid Waste Landfill with a separate Land-Clearing and Inert Debris (LCID) landfill, a white goods management area, a scrap tire collection area, an electronics recycling area, and a pallet collection/grinding area. 2) This Facility is permitted to receive all Municipal Solid Waste, Construction & Demolition Waste, Yard Waste, Land-clearing Debris, Scrap Tires, and White Goods generated within Onslow County. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section 6709_INSP_20160622 Page 2 of 4 3) Proper signage including the Facility permit number, accepted waste types, banned or excluded items, hours of operation, and the emergency contact number is posted at the facility entrance. 4) The Facility is secured by means of a locked gate at the Facility entrance. 5) The hours of operation are Monday through Saturday 6:30 AM to 5:00 PM. Three Holidays are observed: New Year’s Day, Thanksgiving Day, and Christmas Day. 6) The Scrap Tire Management Area is being well maintained at this time. 7) All scrap tires are placed directly into trailers for temporary storage until transported to Central Carolina Tire, permit #43-04 in Cameron, NC for final disposal. 8) An adequate number of trailers are maintained on-site for the volume of tires managed by this Facility. 9) The White goods/Scrap Metal, Electronics, and Scrap Tire Recycling Element was reviewed during this Facility Compliance Inspection. This element of the Facility is being well managed at this time. 10) All collected white goods and scrap metal are being maintained on the concrete pad. 11) Freon containing white goods are being maintained in an upright condition and separate from non-Freon containing white goods and other recyclable metals. 12) Construction activities for the new Onslow County Vehicle Maintenance Facility were underway during this Facility Compliance Inspection. 13) Facility monitoring wells were reviewed during this Facility Compliance Inspection. All wells were readily accessible, properly labeled, and secured with functioning locks. 14) This landfill is currently operating in Phase III. 15) This Facility is approved for the use of Posi-shell as an alternate daily cover. 16) Soil is stockpiled adjacent to the working face daily for use as cover. 17) The working face is being maintained in a compact and manageable size. 18) Windblown litter is well managed and no windblown litter was observed outside of the active working area during this Facility Compliance Inspection. 19) Areas of sparse cover were observed in several areas on the outside slopes of Phase I. *Please seed and mulch FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section 6709_INSP_20160622 Page 3 of 4 these areas to establish a groundcover suitable to prevent erosion of your landfill cap system. 20) Erosion was observed in the soil cover in various areas on the sideslopes of Phase II and Phase III. *Please work to repair all eroded areas by filling and grading at a slope not greater than 3:1. All repaired areas should be seeded and mulched to establish a groundcover suitable to prevent erosion of your landfill cap system. 21) The outside slopes of Phase II and Phase III are in need of seeding and mulching to establish a groundcover suitable to prevent erosion of your landfill cap system. *Please seed and mulch these areas to establish a groundcover suitable to prevent erosion of your landfill cap system. 22) *Please ensure that all outside slopes are maintained at a slope not greater than 3:1 with a cover crop suitable to prevent erosion of your landfill cap system. 23) Construction of the Phase IV expansion project was underway during this Facility Compliance Inspection. 24) Landfill Gas (LFG) is collected and plumbed to the on-site Enerdyne Power Systems LFG-to-Energy plant. 25) The LFG-to-Energy plant was in operation during this Facility Compliance Inspection. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section 6709_INSP_20160622 Page 4 of 4 26) The leachate collection system cleanouts are highly visible, readily accessible, protected, and well-maintained at this time. 27) Leachate is treated and stored in on-site leachate containment tanks until pumped via force main to the City of Jacksonville Wastewater Treatment Plant. 28) The LCID Landfill was reviewed during this Facility Compliance Inspection. 29) No unpermitted materials were observed at the LCID working face during this Facility Compliance Inspection. 30) The LCID Landfill footprint has been marked with permanent, highly visible marker posts delineating the permitted disposal area. 31) All LCID Landfill sideslopes are being maintained at a slope not greater than 3:1. 32) A cover crop of warm season grasses has been established on all non-active areas of the LCID landfill. The cover crop is well maintained and no areas of erosion were observed during this Facility Compliance Inspection. Windblown litter is well managed and no windblown litter was observed outside of the active working area during this Facility Compliance Inspection. 33) No off-site erosion was observed during this Facility Compliance Inspection. 34) *Corrective measures are required as a result of this Facility Compliance Inspection.
