PURPOSE OF SITE VISIT: Comprehensive Facility Compliance Inspection STATUS OF PAST NOTED VIOLATIONS: Resolved OBSERVED VIOLATIONS 15A NCAC 13B .0566 OPERATIONAL REQ. FOR LAND CLEARING/INERT DEBRIS (LCID) LANDFILLS (1) Material (treated lumber, cast iron drain line and 1 large piece of PVC pipe. Pictures attached.) needs to be removed and taken to permitted Construction & Demolition Landfill. Keep receipts for review during next inspection. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS 1) The Operating hours are M-F from 7:30 AM to 6:00 PM. 2) Waste acceptance is to be only by city staff and city authorized contractors. The public is not allowed to use the facility. 3) The permit to construct issued on April 15, 2003 was recorded in the Edgecombe Co. Register of Deeds in Book 1383, Pages 1128-1138 on November 24, 2003. The Permit to Operate issued on April 25, 2005 was recorded in Book 173, Pages 0775- 0780 on May 16, 2005. 4) The service area is Nash & Edgecombe Counties. 5) The access road is well maintained and is well secured by a gate. 6) Drive by inspections are being conducted and recorded. Records are now being put in the box at the gate. 7) A rain gauge is being maintained by the WWTP and provides the rainfall data. 8) Cover appears to be generally adequate. 9) Slopes that will not receive waste should be stabilized with mulch or grass. 10) Records were not able to be reviewed today. 11) Need to better identify site boundary markers. There were some grade stakes that may have been the boundary markers. Will identify this on follow up inspection. 12) Need to have someone on-site for next inspection. I will call a day in advance for scheduling. 13) Will do a follow up inspection within 30 days. Mr. Boone has been contacted about these issues and will call John College when resolved. 14) Digital pictures taken during this inspection and some attached. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
