This memorandum summarizes the topics discussed during the pre-construction meeting for the
construction of Cells 1A and 1B at the Great Oak Landfill (Landfill). The meeting was conducted
May 24th at 11:00 a.m. at the Randolph County Office Building, 725 McDowell Road, Asheboro,
NC 27205. These minute are posted on-line

John Workman began the meeting with some opening remarks pertaining to the overall project,
which will involve several contractors working on site simultaneously. Introductions were made by
all present and a Sign In Sheet was circulated. A copy of the Sign In Sheet is provided in

Attachment A. Mr. Workman stated that Preston Contractors will serve as overall Construction
Manager on behalf of WM, in addition to their role as contractor for Cells 1A and 1B. The
additional contractors working on the site include a paving contractor, leachate tank contractor,
utility contractor, and building contractors.
Following Mr. Workman’s opening comments, Steve Lamb chaired the meeting. Steve stated that
the purpose of this meeting was to discuss the construction and testing of Cells 1A and 1B. A
Meeting Agenda prepared by SCS Engineers was provided to all. A copy of the Meeting Agenda is
provided in Attachment B. Steve indicated Meeting Minutes would be provided to all attendees and
that anytime during the meeting questions could be asked.
The following items were discussed:
1. Personal protective equipment (e.g., hard hat, steel-toed boots, safety vest) should be worn at all
times. Any visitor to the site must also wear hard hat, safety vest, and steel-toed boots.
2. Contractor is solely responsible for the health and safety of their employees and subcontractors.
3. Preston Contractors stated that all people who enter the site must sign in daily to confirm they
have reviewed the Job Safety Analysis (JSA).
4. Preston Contractors stated that a daily JSA meeting will be held at the start of each day. The
CQA Monitor will attend the daily JSA when present at the site.
5. The Owner of the project is Waste Management of Carolinas, Inc. The Owner is responsible for
the overall contract management.
6. The Engineer and Designer for the project is S&ME. S&ME is responsible for site engineering
services related to the design and should be contacted for proposed changes or clarifications.
7. S&ME is also responsible to provide final drawings stamped “Issued For Construction” to all
interested partied. Ming-Tao requested an electronic copy and John Murray requested ½ size
hard copy. S&ME stated a “file share” link will be emailed to all parties.
8. SCS Engineers will be providing construction quality assurance (CQA) services during the
construction of Cell 1A and 1B. CQA services will include the preparation of the Construction
Certification Report to be submitted to DEQ. Atlantic Coast Consulting (ACC) will be a sub-
consultant to SCS and will primarily provide the on-site CQA monitors.

c. CQA Officer/Certifying Engineer – Steve Lamb
9. The Surveyor of Record will be Charlie Morgan Surveying, PLLC. The Surveyor of Record will
be responsible to provide as-built documentation of the cell components, such as the top of
subgrade, top of structural fill, and top of protective cover layer.

10. The General Contractor will be Preston Contractors. The Project Manager for Preston
Contractors will be Dakota Hinterer. The Site superintendent will be Chad Howdershelt.
11. The installer of the geosynthetics for the project will be Environmental Services Inc. (ESI).
12. Ming-Tai Chao, DEQ, stated the following:
a. DEQ will review of site design hydrogeology report and environmental permitting.
b. DEQ will confirm all monitoring wells are maintained and/or replaced and correct
documentation is prepared.
c. DEQ will review all CQA document submittals.
d. DEQ and Local Fire Department will need to be notified prior to blasting, and a Blasting
Plan should be prepared by the Contractor.
e. A pre operations (pre-op) meeting should be scheduled with John Paxton prior to
beginning operation.
f. Per DEQ rule, the liner must be evaluated for leaks – Electric testing.
13. John Patrone, DEQ will be the on-site inspector.
14. John Murray stated he is the DEQ Project Manager and will be responsible for approving the
Construction Certification Report and issued the Permit to Operate. He stated Ming will assist
by reviewing CQA documentation and testing results.
15. Christine Ritter is the DEQ hydrogeologist for the site.
16. Review lines of communication and authority
a. If the Contractor has questions or requires clarification of the Construction Drawings,
either Design engineer or CQA Manager should be contacted to address questions.
b. CQA Manager should be included on all project communications to assist in providing
direction in the field.
17. Preston stated they have reviewed the Drawings, CQA Plan, and Specifications and did not have
any questions at this time. It should be noted that a meeting was held prior to the pre-con
meeting to discuss the erosion and sediment controls.
18. Jason Workman of Preston Contractors provided an overall discussion of the project schedule.
Key items included:
a. A preliminary project schedule will be submitted to all parties in the next few days.
b. Mass Excavation of cells will begin June 24th.

c. Certified base grade inspection is required prior to liner installation.
d. Tentative date for beginning of liner installation is August 26th.
e. Goal is to finish construction of Cell 1A by December 1st.
f. Today clearing, haul road preparation, and temporary stream crossing work will be
conducted.
19. SCS stated the project schedule must be updated by Preston on a monthly basis.
20. SCS stated that two separate Construction Certification Reports will be prepared for this project.
A report will be prepared for Cell 1A and a separate report for Cell 1B.
21. The subgrade must be inspected by SCS Engineer’s Professional Geologist (PG) pursuant to the
permit conditions and NC regulations. SCS will notify Christine Ritter (DEQ) prior to this site
inspection.
22. Daily Field Reports will prepared by the CQA Monitor. These will be reviewed by the CQA
Manager.
23. A Weekly Summary Report will be prepared by the CQA Manager
24. Copies of the Daily and Weekly Reports will be submitted to John Murray, DEQ.
25. Submittals and Shop Drawings shall go to Steve Laskota, S&ME and copied to Owner and CQA
Manager. Responses will be provided from S&ME
26. Geosynthetic manufacturer’s testing data (MQC) should go to CQA Manager.
27. Pay Applications should go to Owner. These should be submitted the 1st of the month.
28. Submit Pay Applications monthly to the Owner with adequate documentation and/or surveys to
verify quantities requested for payment. CQA Manager will assist Owner as needed.
29. Progress meetings will be conducted bi-weekly initially as erosion and sediment controls are
established.
30. Meetings will be conducted weekly as cell construction begins. Locations of future meeting
TBD.
31. A liner pre-construction meeting will be held prior to the geosynthetics installation.
32. Permanent benchmarks needs to be established in the field by Charlie Morgan surveying.
