The Solid Waste Section approved Duke Energy's request for abandoning piezometer CLF24-501 in a letter
dated September 22, 2025, as well as a piezometer CLF24-502 in a separate letter dated September 15,
2025, both associated with Industrial Landfill No. 1 (1812-INDUS-2008) Phase 5.
Duke Energy is providing the well abandonment records for the piezometer CLF24-501 and CLF24-502 in
this submittal. The abandonments were conducted by S&ME on September 23, 2025, and September 18,
2025, respectively. Pre -abandonment groundwater levels are included in Table 1 of the attached report.
The associated GW-30 forms were submitted separately to NCDEQ by S&ME.

S&ME Project No. 21410007, Ph. 02
Dear Ms. Davin:
In support of continued grading operations for the Marshall Steam Station Industrial Landfill, Permit No. 1812-
INDUS-2008, and at Duke Energy's request, S&ME abandoned groundwater monitoring wells CFL-24-501 and CFL-
24-502. The report and the attached information serve to document these well abandonments.
The wells were abandoned by North Carolina Certified Well Driller Jay Little (2717A) and an S&ME geologist in
accordance with the 15A NCAC 02C Section .0113 Standards for the Abandonment of Wells (02C Standards). Each
abandonment comprised the following steps:
If present, dedicated tubing, pumps, or transducers were removed from the well and left on site with Duke
Energy personnel.
Depth to water from ground surface was measured using an electronic water level meter.
Each well was abandoned using bentonite pellets (versus mixing Quik-Grout as access to the wells was
limited at the time of abandonment).
The following documentation supporting the abandonments is attached:
♦ Figure 1, Monitoring Well Abandonments (September 2025)
♦ Table 1, Summary of Monitoring Well Abandonments
♦ Well Abandonment Records (GW-30 Forms)
As required by the 02C Standards, S&ME submitted the Well Abandonment Records (GW-30 Forms) to the North
Carolina Department of Environmental Quality (NCDEQ), Division of Water Resources (DWR) Information Processing
Unit under separate cover.
