The report and the attached information serve to document these well abandonments. The wells were abandoned by North Carolina Certified Well Driller Jay Little (2717A) and an S&ME geologist in accordance with the 15A NCAC 02C Section .0113 Standards for the Abandonment of Wells (02C Standards). Each abandonment comprised the following steps: If present, dedicated tubing, pumps, or transducers were removed from the well and left on site with Duke Energy personnel. Depth to water from ground surface was measured using an electronic water level meter. Each well was abandoned using bentonite pellets (versus mixing Quik-Grout as access to the wells was limited at the time of abandonment). The following documentation supporting the abandonments is attached: ♦ Figure 1, Monitoring Well Abandonments (September 2025) ♦ Table 1, Summary of Monitoring Well Abandonments ♦ Well Abandonment Records (GW-30 Forms) As required by the 02C Standards, S&ME submitted the Well Abandonment Records (GW-30 Forms) to the North Carolina Department of Environmental Quality (NCDEQ), Division of Water Resources (DWR) Information Processing Unit under separate cover.
